How to strike spike in post prandial blood glucose of an insulin-dependent type-2 diabetic with near normal fasting level 91 mg/dl and

Thanks. My pre-brunch, as per my Endocrinologist's advice, (at 9.30 am) insulin (mixture of insulin aspart and protamine crystallised insulin aspart - 30% & 70%) being 26 units and pre-dinner (8.30pm) 16 units, aside from 1 tab. of Januvia (Sitagliptin Phosphate) 100 mg -pre-brunch.Post-brunch I use 1 tab. of Regly (Biotin Minerals) & Clavix (Clopidogrel) tab. 75 mg. In the afternoon, around 1.00 after my snack, I take 1 Centrum (multivitamin) tab and 1 fish oil cap. Post-dinner I've 1 tab. of NephroSave and 1 Polycap (Ramipril 5 mg. etc).

As for my full day's diet, immediately after my 45-minute walk I have a cup of coffee without sugar, around 7am. At 9.30 I take my brunch (not breakfast, based on the doctor's advice) 75 gr organic quinoa with greens/veg curry plus buttermilk. Around 12 noon I take 6 almonds and 3 walnuts/pistachios/pecans and @ 1.00 pm I 've a snack ( 3 idlis or 50 or wheat rava)

Around 3.45 a cup of tea w/o sugar. and around 5.30 pm oatmeal with diluted milk - 2% fat - and a fourth of an apple. Around 8.30 pm I 've my dinner with 2multigrain phulkas + vegetables and 40 grams of quinoa with buttermilk.

I'm 75 and 've been a type-2 diabetic for 33 yrs and insulin dependent since 1998.

There are two ways you can control pp sugar. 1. Chew voglibose tabs just before lunch/dinner 2. Reduce carb intake. Get rid of idlis, fruits, roti etc and add boiled egg to your diet.

Your description is incomplete. you have to specify type of insulin, ppbs and your HbA1c

In our body there are two type of cells for insulin production , one for the 24 hours production (BASAL insulin) and other for to compensate your food intake ( BOLUS INSULIN ).

It seems that your BASAL insulin is enough but your Bolus is not enough to cover the food Or you are taking more food.

If you are on Mixed insulin you may have to increase to your bolus dose ( May have to change your insulin)

If you are in a Analog insulin you have to start a short acting (BOLUS) insulin with each meal.

Here i mentioned are few possibilities only But You should have to consult a doctor for your dose titration and Change of insulin.
