B1 Overdose

I need a bit of advise, pls. I think my HwP has overdosed on 500ml hcl daily after taking just 2 weeks. His symptoms are more noticeable especially his balance and speech. We plan to take a break from the B1 to ascertain if he’s overdosed or not. In the meantime, should we wait for the symptoms to clear? Then start on B complex & Magnesium Citrate for two week and back on reduce dose of B1

I have not heard of symptoms getting better in the event of overdose. You already know what you need to know. Time to take a break.

Dr. Costantini's preference was to start B1 alone until an optimal dose is found and the benefits are improved and stable with a good pull test, before adding other supplements that may alter your results. Other supplements during the testing period can confuse the issue.

Stopping until he returns to his pre B1 base and then restarting at a lower dose seems appropriate. Are you taking the videos to document his pre B1 state compared to his post B1 state?

My understanding from Parkinson's B1 therapy page is that , and different from what is in Daphne's book, now they recommend starting Magnesium and B1 complex ( with low B6) 2 weeks prior to commencing HDT therapy.

I was stating Dr. Costantini's experience in roughly 2500 patients. The use of B1 is still evolving, but the website that he started states this :

' However, we do not begin the therapy with high dose thiamine in addition to multivitamin compounds, especially if these contain vitamin B6. This because vitamin B6 is a facilitator of the peripheral decarboxylase and in PwP may interfere with the amount of L-dopa that reaches the brain and thus worsen the symptoms. '

On his FAQ page he states this in question #33 :

33. If I need to stop taking other supplements to start thiamine therapy, when can I start to add them back?

Q: It seems that your preference is for most people to stop use of all supplements in order to start thiamine therapy and I assume this is so that you can have the clearest picture of exactly how these patients are responding to the thiamine therapy itself and in turn this helps you to determine if dose adjustments are needed? You are also trying to make sure that the other supplements are not clouding the effect of thiamine therapy?

If this is correct, at what point can a patient start to add back supplements they were already taking as part of their daily regimen?

A: That is correct. We usually prefer to have a glimpse of how the high dose thiamine performs alone before evaluating the need for further supplements to aid its absorption (e.g., magnesium, etc.).

Usually, after 3 months from the beginning of the therapy, we reach the point where the thiamine protocol has been correctly tailored to the needs of the patients and in case the regression of the symptoms still leaves room for improvement, we add other supplements and observe possible changes for further 3 months.

Often times the action of thiamine alone is satisfactory for the patients and rarely we need to add other supplements, which are in any case in small doses.

This is his protocol as he stated it and I realize that other people do other things, but that would no longer be his protocol, but rather something else.

Art, thanks for your response. I read from Dr. Costantini's finding to add B Complex & Magnesium after one has found their sweet spot. However, from reading Daphne’s book & listening to Elliot’s video on YouTube it was recommended to start B Complex & Magnesium 2 weeks before introducing B1. This was why I asked the question. Now that we are taking a break, not sure which way to proceed after the break.

Yes, I’ve documented his pre state before the B1 therapy and his post state couple of days after B1. I know it’s working for him cuz we’ve had couple of guests come over and was very impressed with how he was looking. The poker face wasn’t there as much, he was able to make conversation clearly. One even said “whatever you are doing, pls continue”

Either way, it is clear that B1 is going to work for him and both ways get B1 to him in a useful way. The thing I find to be most important in his case is that he is a B1 responder and it is worth appreciating that fact.

The thiamine mononitrate dissolve in the mouth tablets seem simpler since you only have to do one a day and they are fairly inexpensive. Whichever direction you choose, it is good to know that he is going to respond to B1. I don't go on Facebook and I haven't read Daphne's book, so I can't comment on that method.

all these replies has made the b1 therapy sound very complex and challenging when in fact it is very simple and easy to follow. If you have been on various supplements for a long time stay with them. Then choose to add a recommended bcomplex and chosen magnesium from recommended list. Take for a month before adding b1 . The alternative is start b1 first, find your sweet spot THEN add bcomplex and magnesium. Monitor and record every tiny change or improvement. If you hit OD symptoms stop for two weeks and start again with a lower dose. Read Daphnes book from Amazon for full details and watch her utube videos

Things have moved on a bit from the early days of Dr Costantini's B1 Therapy. Daphne now suggests taking the B complex and Magnesium before the B1. My brother started on B1 first but overdosed on 500mg a day, so he stopped and started the Magnsium and B complex, just make sure the B Complex has a low dose of B1. After stopping for 2 weeks he started at 250mg agin he overdosed, he kept up with the Magnesium and B Complex and another 2 week break, he's now taking 100g B1 whci could possibly be too low, but he's on day 4 and doing well, sense of smell is back he's waliking upright and other things are improving.
