Can anyone give me advice on hallucinations. My partner just started on 100/25 c/l 3 times a day and has been hallucinating intermittently ever since... moreso at night. They aren’t troubling hallucinations so he’s coping fine but I worry that they’ll become more difficult to distinguish from reality. Is it because he went from only taking Amantadine to taking both Amantadine and Sinemet? Will they subside? Any recommendations are most welcome!! He recently got the first Pfitzer shot and symptoms have definitely worsened. Hoping this setback doesn’t last too long!!

How long ago was he diagnosed? To start hallucinations early in PD makes me wonder whether he actually has Lewy Body Dementia. Be sure to report these symptoms to his PD doc.

The diagnosis came last summer but looking back I can see it was a late diagnosis. The prescription for Amantadine came first in January; he has significantly deteriorated since then. The Levadopa seems to quell the tremor only a little and his balance is only good in the morning... falls (backwards) have increased and in addition to hallucinations there’s intermittent cognitive impairment.. he can still play chess when on but is often forgetful; anyway hoping to talk to neurologist tomorrow!!!

Talk to the neurologist.

Sometimes Amantadine can trigger hallucinations. But it also may be that the sinemet dose is too high for him.

sorry to hear! there are a handful of hallucination posts on this forum... if you search them up they might help... they have helped in understanding hallucinations and how to possibly solve this issue. one thing that i learned is that it is either signs of dementia or the meds. when my dad was experiencing hallucinations, the neurologist's solution was more drugs, another kind of prescription. we didn't go that route. instead we reduced my dad's neupro patch and his hallucinations went away. others on this forum have used niacin to help with hallucinations and this has helped them greatly. best of luck, hope you can figure it out soon

Thanks so much for taking the time to reply!!! It’s fairly concerning... I’m pretty sure it’s the drugs because it wasn’t happening before he started Sinemet. I’m really hoping to solve it soon ! Thanks again.. I looked at some of the posts but I will read more.

Another thing that helped my dad a lot was transitioning from mostly Sinemet to Mucuna. Not sure how much you know of it but it's a naturally occurring dopamine with other neuroprotective properties. Worth checking out! Also a lot on this thread about it.

Update. Phew, after reducing his dose to 1/6 what he had been taking (today he started a half dose of the immediate release Sinemet 100/25 taken twice daily) he was cognitively sharp today! And only a few very slight hallucinations... nothing compared to the delusions of the day before! Hoping to land on the right dose for him. Like for many others, Night time is hard. But man was it ever harder with those cognitive disturbances! Also stopped the Amantadine. Hopefully the shock isn’t too hard on him tomorrow... today was a pretty good day compared to the previous 4. That Pfizer shot really set him back I think, but he really had no choice but to take it due to the COPD compromised lungs. I greatly appreciate the support here! Thanks so much !

hi Scrudgie! how's it going with your partner and his hallucinations? my dad started on c/l immediate release (sinemet) 2x and then bumped up to 3x a day and is now continuously hallucinating for the past 3 months.... he initially had an agonist (mirapex) added in the beginning 2x a day then uped to 3x a day as well, but since the reporting of hallucinations the doc discontinued the mirapex and hallucinations are still here... I notice this post was 2 years ago, so i wanted to check in how have you and your partner adjusted his medication regimen since? especially for the nighttime. can drug-induced hallucinations actually go away?

I believe nobody has mentioned this. Has he tried melatonin, 5 mg before going to bed? It might be helpful.
