The issue in the future will be how and if an extraction process can be developed that is efficient. So far, we are years into its development, and they still will need stem cells to make it.
Because of their transfer capabilities with mRNA proteins, perhaps they also have application to Covid-19. Who knows?
Exosome preclinical research and clinical trials are being conducted worldwide. There are currently no clinical trials for exosomes for PD, just preclinical research (rat & in vitro experiments). There are clinical trials for other diseases, though.

Stem cell clinics in China are racing ahead with trials and I hope they are conducted ethically and produce reliable results, but they do seem a bit sketchy since some of the regular stem cell trials listed in clinicaltrials.gov for these clinics seem like adverts. Beware of clinics claiming to offer exosome therapies right now that have not gone through the clinical trials process:
