MyBeloved has taken madopar capsules at maximum dose daily for years. What happens next?

what consultant told him was maximum dose is 5x2 capsules (one is cr). If you reached the end of effectiveness what were you offered as choices?

Most likely a dopamine agonist which has several potential serious adverse effects. Time to get serious about exercise, thiamine, etc. if he hasn't already.

Opinions on max dose vary. Many take quite a bit more than that.

Depends on your loved ones age and stage. I see your husband has been diagnosed 22 years. You will have a wealth of knowledge to share! Has he been on madopar all that time and is he dyskinetic? So many people have such a fear of madopar or sinemet but I'm guessing it has been helpful for your husband.

Many neuros won't prescribe agonists as a new med after about 65yrs because it can be a hard med to tolerate. But anyway you husband is on the neupro patch so he is already on an agonist.

The skill in PD treatment is about getting both the amount of meds and the timing right. So the neuro should work with you to adjust meds and timing to give the best coverage. There is no maximum dose, that sounds like the maximum your neuro feels comfortable prescribing.

Diagnosed at 72 - in 1996, is that correct? Sounds to me like an amazing story you two must have to tell!

If I read that right he is 94 years old and has had PD 22 years. I wonder if he is worried about disease progression and a long life.

I'm currently taking 6 x 125 Madopar per day plus a slow release at bedtime, diagnosed with PD 10 years ago, lately been having freeze ups due to short window when Madopar seems to be effective, but that window is getting smaller, cant really offer any other advice on Madopar but very grateful to have come across this site, have been in contact with Dr C and starting HD Thiamine next week, here's hoping that there will be some improvement.
