So I started on 4g of B1 hci 5 days ago. 2g 2x a day. It seems my symptoms are worse. I’m female 5’8” 155 pounds, very mild PD, mainly right hand tremor. It seems I’m tremors g more and having foot weakness. I take 50/200 C/L er twice daily. Should I stop the B1 for 5 days then cut down to 2g per day?

Did Dr. Constantini recommend 4 grams per day? I emailed him and he only recommended 3 grams per day for me. I was just diagnosed, have very mild symptoms and am a 198 pound male. Four grams seems a lot for you.

I'm curious about when to stop taking Sinemet when starting the 2-4 g/day thiamine regimen. I only take (2) 25/100 Sinemets/day, and sometimes that seems too much. I'm interested in the exercise and MAOb inhibitor (Selegiline) to slow my minor symptoms.
It's sort of a hybrid approach as per John Pepper and Dr. Constantini. I'm first getting my blood test for B1, and then talking with my neurologist.
Be extra careful in titrating down Sinemet.
Neurologists adjusts meds by big leaps.
Pls.read ONCE UPON A PILL by Janice Hadlock. Her article csn be downloaded online for free. Long read but loaded with experiences of pts. related to not being careful in adjusting meds. I think her recommendation the 10% rule. 10 % every 10 weeks increase or decrease.

Since you plan to have a thiamine blood test, it is worth mentioning that Dr. Costantini has found that in his experience at his office in Italy that his patients have generally shown thiamine levels that are in range and once started on HDT, those same tests are off the charts.

You should definitely email dr Constantini. The dose can be different for everyone and may not be as straight forward as following what other people are taking. If you google translate your email into Italian the response is quite quick. It’s not hard to do and much better than guess work. You will find the right dose quicker with good and very experienced advice. There is no need to try and work it out on your own. What is right for others might not be right for you.

I am sure someone posted his email already, but if someone would be so kind as to do it again, I would appreciate it. I want to write him, too, for suggestions and dosing and will ask one of my docs to contact him as well. (I think in the interview that was suggested, so that the word gets around and so our PD docs know what we are doing and to keep an eye out for possible issues.

After hearing the interview of the MD in Italy by Robert Rogers at Parkinson's Recovery a week or so ago, I'm very interested in this topic. My tremors and neuropathy are worsening by the day, which is quite scary, and ergo I am open and willing to try new approaches (many of which I have tried over the last 15 years).

Did you write to Dr C? He will tell you. I believe there is a scale according to your weight.Then it get adjusted if its not right.
