Hi all im currently diagnosed with cardio myophy, anxiety,numerous ventrical hernias, diabetic t2, all severe and at the moment no reviews for 2 years.im about to have a pip and im very distressed and concernd that the most significent complained isnt visual.
Can anyone give me some advice my LV refraction on my left side of my heart is less than 10% so most mundane tasks are able but exhausting or devastating after the fact i dont walk anywere as moving on my seat exhausts me. Thank you

On the form that DWP sent you just gather as much information on how this condition is affecting your daily life.
How you express this on the interview will be crucial as you have to give a true picture of your struggles. Take your prescription with you if you are on medication and the last hospital or doctors letter.
You have nothing to worry about. They just want to know more about how the condition affects you really. No need to panic.
Best advice is get a copy of your medical records from the doctor. I did. Also ask a member of your family and or close friend who knows you well to help fill it in. You are allowed to have that person add to the forms and go in with you for your assessment. It worked for me. Don't hold back,write everything down on how it affects you. I got enhanced on personal care and mobility.
Make sure that you have a good supporting statement from your GP or ideally specialist (even if you have to pay for it), and make sure the statement itemizes the ways in which your CMT etc etc etc affects your everyday life (even if you have to dictate the letter to your GP or write it for them to transcribe. I have done this many times now). Don't panic but do take this very seriously...
