Thanks for that. I especially liked the comments regarding 'one size does not fit all', and the confirmation that even 'well meaning' specialists can disagree on the best treatment options...

I guess that as time goes on and more data are collected in clinical trials, the answers regarding which treatment to choose will become clearer...
