44M, dx T2DM in the summer, went into remission with an hbA1C of 37 having lost 13kg and changed his diet, heads to the gym twice a week. He's done good! He was also dx with NAFLD at the same time but his platelets kept dropping. Sent for an ultrasound. Spleen found to be 17cm. Platelets dropped yet again and MCV significantly raised, though LFTs have remained within normal limits. Referred to GI, Fibroscan was yesterday.
Result was 47.7kPa, the highest readings were around 75. The nurse got quieter and quieter as the scan went on!
He's now been prescribed Carvedilol for portal hypertension (dx through the fact his liver is fried and his spleen size, and certain bloods). He's fast tracked for an urgent consultant appt and I imagine they'll want to do an MRI. For 6 monthly U/S obviously and 6 monthly consultant and nurse led appts time about.
He's currently asymptomatic and so obviously compensated.
This has come out of nowhere. He is absolutely convinced he's going to die. Not in a dramatic way, he just has this sense...
Who else has been in this position and can you give me any insight as to how it went/will go down? We have 2 boys 12 and 14. Our brother in law died of HCC a few years back after a cirrhosis dx - same trajectory, T2DM, cirrhosis, HCC and died all within six months. I just want to be prepared for the hard.
*Please: no diet fads or whatever. I'm not in the head space for that. He's doing and has been doing everything he should be. I just need realism*
