MELD score

Afternoon,I am trying to understand my husband MELD score but can't find INR results in his bloods.

Could it be serum inorganic phosphate?

I used this to work it out and it comes out at 54.

This is alarming for me as he isn't due to speak to hepatologist until June.

Please does anybody know?

No, the INR is international normalized ratio (INR) / Prothrombin time (PT) i.e. is the time taken for blood to clot not the result you've used which is not a factor in calculating either MELD or UKELD scores.

MELD (Model for End Stage Liver Disease) score only goes up to 40 - the more commonly used one in the UK is UKELD (United Kingdom Model for End Stage Liver Disease).

It doesn't look like that has been tested. Would that only be tested by the liver consultant?Thank you Katie x

It's certainly a test hubby always has done during his consultant run tests as it needs special tubes to prevent it clotting before reaching the lab. You are better leaving the calculation of MELD/UKELD to the experts as some of these online calculators also use different units of measurement than blood test results show. We have NEVER had a copy of any of hubbies hospital bloods and only ever one set off the GP in almost 11 years of diagnosis.

I am able to see all his test results in my NHS app, because his is linked to mine.I have proxy access.

I guess a little information can be scary if we arent sure what they mean.

I do worry because his sodium was very low and his CRP and platelets were high.

He doesn't eat and sleeps an awful lot.

He still drinks over 20 bottles of beer a day.

He was in hospital a few weeks ago for a sodium IV.

Thank you - did you ask the GP or hospital / consultant ? My NHS app says to contact my GP surgery

I get my bloods e-mailed to me every time I have them done just ask your doctor's liver nurse she sorts mine out

I rung the GP receptionist and can now see all my test results on the NHS app. They did it straightaway

Thanks will try that

You can figure out your MELD here..

But you need the INR.. Mine was 1.15 at last check.. You can plug that is as a barometer if you want..

hepatitisc.uw.edu/page/clin...

Does he bleed easily? Or bruise easily? If so that shows the blood is taking longer to clot.. When I was first diagnosed I tripped and fell running to catch the last train out of New Jersey to get back to Manhattan.. I had a HUGE black freaking bruise on my thigh for like 7 months.. It wouldn't go away. Shortly after that they said I had cirrhosis..

On the upside my INR is normal now. :)

He ain't gonna get better unless he lays of the booze.. Took me about 2 years to finally submit and gave it up.. Life's a lot better than feeling like shit all the time.. Give me my normal liver back and I'll go buy a bottle of wine.. But that ain't gonna happen so it's seltzer water and an occasional 0.0% alcohol Heineken which tastes remarkably like the real stuff.. I shouldn't be drinking that either but once in a blue moon it scratches my itch.

Oh.. And the concert sucked.. I was dragged to a Styx concert.. In f'n New Jersey. And I didn't get laid.

Cheers me dear. I used 1.15 and he scored 33. Sounds better than 54, but still not good.Well done you for staying off the booze.

Please may I ask, what inspired you to stop?

X

in reply to Diamondbabe

Spitting up blood and retching like I was going to die..

in reply to Diamondbabe

He's also still drinking. Lay off for 6 months and I bet most numbers will be normal.. On the MELD I think they start looking at liver transplant at 15 or so. But that's just starting to look. And you won't get on any transplant list while drinking. Or smoking weed or drinking non alcoholic beer.. And here I believe they make you go to AA (although that I would probably refuse to do)..

Livers are hard to come by. They don't just piss them away on someone who they think will drink again. Here you can also list yourself in all 50 States.. Steve Jobs did that. Prick.. lol

I can't see him stopping imo.I have tried so many avenues for him.

He won't admit he's addicted.

He's in a right state. His spine is fractured in 4 places. He shuffles around the house on a wheeled support. Sleeps in his clothes and never washes.

He's nothing like the well groomed man I married.

I get angry, then upset.

It's such a waste of a life.

What's wrong with alcohol-free beer ?

in reply to AnxiousPete

Well. Most of them have some trace of alcohol in it. That's a no no.. But imo they feel like the NA stuff will be a gateway back to the real stuff. And they are probably right. Hasn't happened for me so far. I am perfectly fine with my 0% Heineken and .05% wine.. If they would allow that I'd be good for life..

I never drank to get wasted.. I just LOVE freaking wine.. Alcohol or no alcohol..

AnxiousPete profile image

I love wine but I find unlike the beers I don't think any of the AF ones taste anything like wine :-(

in reply to AnxiousPete

They have one called Fre that Sutter Home makes.. Not great but the Chardonnay is drinkable. And I had a good one from New Zealand but that one was like $15 a bottle.. I'll try and find the name. But you should lay off of all of it anyway.. I have to say that. :D

If you are ever likely to end up a potential candidate for transplant and your cirrhosis is due to alcohol then use of non alcohol alternatives to real alcohol drinks is a no, no. They are as much contraindicated as the 'real thing' in that they don't distance you from that desire to drink, they never match up to the real thing and they are seen as a potential trigger to a return to unhealthy ways. People who are listed for transplant can be delisted if there is a 'slip' in their abstinence.

I only was a 5 or 6 glass of wine a night drinker.. More that I should have but I never thought it was that much. Turns out it was.

When your husband starts spitting up blood that might slap some sense into him. Assuming he hasn't already. The concept of death is easy to overlook. Until it's staring you right in the face. Different people have different breaking points. And I guess some don't have any at all..

Hang in there. He hasn't got scared enough yet.

Does spitting up blood indicate varices?

Yes. I had a couple of esophogeal varices and one small gastric that they banded. Not a lot of blood but I was spitting it up.. Previously I had bled when brushing my teeth due to low platelets and the high INR/Pro Time. I cut near my nose once while shaving and it looked like the OJ Simpson murder scene.. :D

My blood wasn't clotting fast enough..

Diamondbabe in reply to

He has been bringing up small amounts of blood when he is sick, which is normally if he hadn't drunk though. He does bruise very easy too.

Sounds like you realised in good time to give you a fighting chance.

I just say you are very strong willed to do so.

Honestly.. It really wasn't that hard.. I just stopped and that was it.. I'd still like to have a glass of vino but I have to make due with other crap. lol I got a ski trip coming up.. That's when I will miss it.. Shirley Temples on me. :D

Sorry to be blunt here,. Don't worry about the semantics, just stop drinking alcohol. The appt / blood tests in June will make more sense. In the mean time I would really recommend speaking with the BLT nurses. They are superb listeners and have a vast amount of knowledge regarding liver disease, mental health and support strategies. All the best.

As I understand MELD 40 is tops. It is a special blood test. Mine runs at 9.

in reply to PollyO

MELD isn't really a "blood test".. It's just a composite of 5 or 6 numbers they have already taken on your CBC and Liver Function test.. Always ask them to include the INR and Pro Time when they draw blood. That's the one they rarely do and you need that one to compute the MELD score.
