I’ve had deranged lfts for a few years but my recent results led my GP to do a full liver screen with auto antibodies and hepatitis panel.
This was all negative. My abdominal ultrasound was also completely normal with no evidence of fatty liver or gallstones.
My ALT was 191, ALP was 175 and GGT was 92.
My question is what happens next. It seems they aren’t doing any further investigations as my ultrasound was negative but my blood results remain high.

I had years of raised ALT. Like you, I had a negative ultrasound and blood tested for viral hepatitis - also negative. I was then referred to a hepatologist and after lots more blood tests and a biopsy, was diagnosed with autoimmune hepatitis. Not suggesting that's what you have but I would ask for a referral. Best to get it checked out thoroughly.
Just fatigue and muscle pain from what I remember. It grumbled on for years with fluctuations in LFTs before diagnosis. When the hepatologist ran a test for immunoglobulins they found they were high which pointed towards AIH. I don't want to worry you though - you may not have an autoimmune disease at all but it's always a possibility. I have a history of autoimmune disease.
Get a referral to a specialist if need be and if possible, make sure you get to the bottom of things. GPs are generally not qualified to diagnose or treat liver issues.
Do not let doctors fob you off. Despite what some commenters on this forum might say on other threads, you cannot always blindly follow every doc--if one doesn't feel like finding what is going on with you, find one who will. You have to be your own best advocate, your health is too important to ignore. Good luck.
