Very best wishes.

To lower triglycerides cut back on your carb intake & eat more protein & vegetables. Whenever I've done a keto diet eating very low carb my triglycerides have dropped to an amazingly low 0.5 mmol/L. This effect happened each time I went on a keto diet to lose some weight. As soon as I resumed eating carbs my triglyceride level more than doubled but was still well within the normal range. I ate mainly lean meat & vegetables & no carbs when this effect happened.

People with liver disease are encouraged to eat frequent small meals but they still need to be careful with what they eat & how much like anyone else. An example would be eating beans rather than a sugar laden snack that spikes blood sugar levels. Most days I eat a can of a '4 bean mix' I buy at the supermarket & I find it very filling & it satiates well. My neighbour recently attempted to give me chocolate Easter eggs & a packet of what appeared to be sugar laden pastry as a gift & I told him to keep his gift because it wasn't real food & I wouldn't eat it.

A keto diet isn't sustainable long term & people with liver disease need carbs to maintain glycogen levels in the liver for a ready supply of energy. A diet like the one in the link below would probably achieve the same effect as a keto diet in lowering triglycerides but in a completely different way through eating starchy carbs with a low to moderate calorie density & glycaemic index. You could add a little lean meat to it & it would still be healthy in my opinion.

My HDL is 26 and LDL 87, but I’m a smoker too!! I drink 2 tablespoons of ACV with 8oz of cold water daily to lower my triglycerides and flush out my liver

Doesn’t sound like you have cirrhosis, just a plugged up fatty liver, usually the first stages of liver disease is high triglycerides and low HDL cholesterol, often times you’ll see an elevated GGT as well, abstain from alcohol and go to a plant based diet, the liver can heal in 6 weeks if you do the right things

Elevated sugar is par for the course, your liver isn’t sensitive to insulin right now, it’s fatty, if you can get the fat out of your liver, insulin resistance should reverse itself, hence, the high triglycerides, it all goes hand in hand
