Ramipril

Hi my first post so sorry if this sounds a bit of a silly question but am new to medication I am on a few after STEMI have been on them for 12 weeks now and the Doctor has now prescribed another for blood pressure Ramipril, this was done via a nurse the pack does not say AM or PM have looked at the NHS website says can be taken either but may cause dizzy if taken AM, just really wanted to ask if anyone takes at night as that would work better for me, but read that blood pressure gets low at night. I understand that everyone is different but thought it might be nice to get an idea. I take statin and ticagrelor at night and ticagrelor and aspirin in the morning.

NOT a silly question! I know what I would do in your position. I would take it at night for a week and see i felt ok. In the meantime, during that week, i would chat to my dispensing pharmacist to see if he has a view.

Actually, I find it more convenient to take my pills in the morning so I might just dedide to do it that way round

Also, the website of Blood Pressure UK might be a good read.

For what it’s worth I also take my Ramipril in the evening along with Avorstatin. My BPM does lower overnight but that’s normal. I don’t feel the need to take it at an earlier time.

Hi great to hear that, seems a few people take evening with statin, my worry was BP lowering overnight but I guess depends on how high or low BP is normally, I dont Know about my BPM at night but this AM was normal for me since heart attack am sure it will lower once in my system

if it helps this is what my Ai suggests.

Ramipril, an ACE inhibitor used to treat high blood pressure and heart conditions, can generally be taken either in the morning or at night, depending on individual circumstances. However, research and practical considerations suggest that taking Ramipril at night may offer additional benefits for some people.

Reasons to Take Ramipril at Night:

1. Better Blood Pressure Control:

• Blood pressure naturally dips at night, and for some people (e.g., those with hypertension), this “nocturnal dipping” may be absent.

• Taking Ramipril at bedtime may help lower nighttime blood pressure and restore the natural rhythm, potentially reducing cardiovascular risks.

2. Improved Cardiovascular Outcomes:

• Studies like the Hygia Chronotherapy Trial (2019) found that taking blood pressure medications at night significantly reduces the risk of heart attack, stroke, and other cardiovascular events compared to morning dosing.

3. Side Effects:

• Ramipril can cause side effects like dizziness or fatigue, which may be less bothersome if the medication is taken at night.

When Morning Dosing May Be Better:

• Daytime Blood Pressure Spikes:

• If your blood pressure tends to be higher during the day, your doctor might recommend taking Ramipril in the morning.

• Personal Routine:

• If you’re more likely to forget a nighttime dose, morning dosing may be more practical.

Key Takeaways:

1. Nighttime dosing is often preferred for Ramipril to optimize cardiovascular benefits and align with your body’s circadian rhythm.

2. Morning dosing may be appropriate if you experience daytime blood pressure spikes or if nighttime doses cause issues like nocturnal coughing or other disturbances.

3. Always follow your doctor’s advice and avoid switching the timing of your dose without consulting them.

If you’re unsure about the best time for your specific case, consider monitoring your blood pressure at different times of the day and discussing the results with your healthcare provider.

Hi, I've been on ramipril 5mg daily for 9months (and amlodipine), I don't have specific in structures either apart from 'one daily'. I take mine first thing in the morning & it doesn't make me personally dizzy. I understand the most important thing is to be consistent with the time of day. 🙂

I take my small dose ramipril in the mornings no issues. NHS says “You'll usually take ramipril once or twice a day. Your doctor may suggest that you take your first dose before bedtime because it can make you feel dizzy. After the very first dose, you can take ramipril at any time during the day. Try to take it at the same time every day.”

I have been started on Ramipril since only Monday with the advice to take it in the evening. No apparent side effects so far and BP, which is being monitored twice daily, seems OK.

Hi thank you for response I started Tuesday was a bit all over the place but Thursday did take evening and phew so far so good, hearing this just makes me feel so much better it gets a bit confusing, great that no side effects for you so far, I have been asked to take BP at home twice daily so have just purchased a BP monitor

Hi.I take it once a day .It says on the pharmacist label on the box to take it in the morning.

I checked when I had my medication and BP reviews and was told morning .

Looking at Goonerboy's info , I'm wondering if that was based on my BP readings.

It might be worth asking your pharmacist.

I was told to take Ramipril in the evening, and the beta-blocker Bisoprolol in the morning. My blood pressure is usually quite low, and taking the two medications at different times helps manage the effect they both have in lowering blood pressure further.

It depends on the person I think. I am one of the weird ones who had large spikes in my BP in the middle of the night when I was doing nothing so I now take my BP meds and hour or so before going to bed including Ramipril when I took it. For someone very active it might be better to take it in the morning.
