If you have been seen by a GP you may have been prescribed medication. If you haven't seen your GP then perhaps you should. Otherwise salt reduction can lower BP, and of course body weight control, plus managing coffee and alcohol intake, although regular and varied exercise may unfortunately be restricted in your case given what you have told us. Finally there is a lot of time between now and October to sort out a BP management plan including perhaps any meds so don't get stressed about it since will likely only make things worse. And finally finally you will of course have to share your new health condition with your travel insurer if you have not already done so.
I think that as long as the high BP is being well controlled you won’t have any problem. I travelled to South America with all my ailments a few years ago and was fine but I did book business class for comfort! Good luck.
Compression socks are essential and regular movement of legs. If you can't move them is there someone who could do it for you? Do not get dehydrated. Give yourself plenty of time and avoid stress. Tell your insurers. You should have medication by then and have it under control but you still have to tell them. You shouldn't have any other problems.
