Evening all!

Hope everyone's having a lovely weekend!

Received a letter off cardi noting my diagnosis-

JVP elevated 4cm with tricuspid regurgitant waves. Grade 2/6 mid systolic murmur at the apex and left sternal edge. 1st degree AV block and Wenckebach with pauses lasting up to 6.53 seconds.

Told to stop flecainide and bisoprolol immediately.

Any chance any one can shine any light on either diagnosis as its all becoming a bit overwhelming!

I'm really sorry you are having problems and I cannot help you with your query, but I am sure I would feel overwhelmed too. I'm sure someone will come along to decipher it all for you.

I've got a vague idea on a few but it's getting too technical 🙈 I received it by corresponding letter as he sends a copy to my GP each time as well as my Obstetrician. I've got an immediate app on Monday to see Obst regarding the baby as I'm now 25 weeks pregnant. I'm just waiting for an app for another 24 hour ECG since coming off tabs and then ill be in to see Cardi ASAP as he said Wednesday I need a pacemaker. One foot in front of the other at the moment I suppose so appreciate any positive thoughts x

Crumbs! It reads to me as though your letter was intended for a cardiologist, not the patient. Keeping the patient fully informed is one thing, but sending them a load of gobbledegook doesn't exactly fit the bill. Why do these things always arrive through the letterbox at the weekend as well? It's really unhelpful timing.My advice (I'm not medically trained) is not to attempt to work this out using Google; it will very likely add to existing confusion. I think you should get on the telephone to your cardiology clinic first thing Monday morning and ask them to explain these baffling results to you in plain English as well as tell you what next steps they're planning if they haven't already. I expect they're in the process of setting up an appointment for you, but it's worth checking.

Take it easy meanwhile; feet up and plenty of tea!

No no my cardiologist always sends me a copy of the letter he sends to my GP, he always has done since being under him (6 years). He said when I spoke to him on Wednesday just gone if it's not this Monday I see him it'll be Monday after until then I'm to have another 24 hour ECG 🤷‍♀️ x

I’d want to know too! If you don’t feel like waiting for your Cons Appt, I’d call your GP. They will interpret the detail and explain what it means, I’m sure.

I’m not medically trained but having dealt with both my husband’s and my heart for some years I can shine a small glimmer on the jargon, not diagnosis, prognosis or possible treatment options:-

JVP = Jugular venous pressure - the pulse in your neck which can indicate various things which you will need to talk to your GP/Cardiologist about.

Sounds as though it is elevated because (????) of the tricuspid (one of three main valves in the heart)

The systolic murmur is the sound the valve makes when there is regurgitation so the writer is explaining what they hear and where they hear it.

1st degree AV (Atrial, Ventricular) block means that the electrical signals aren’t reaching your ventricles (lower part of the heart) as they should should. Sorry, not a clue what Wenkebach diagnosis is (name will be of the person who first wrote about it) but I would imagine describes the type of block which is causing the pauses in the heart.

I don’t think this type of patient communication is either helpful or ethical without prior explanation. I think you need to have a consultation with your cardiologist and ask for a full layman terms explanation.

Purely going from my husband’s experience - I would imagine that they will want to talk to you about Pacemaker implant, that worked for my husband very for the last 10 years when he was having pauses of up to 9 seconds.

I think it's standard practice nowadays to copy the patient into the correspondence. At least I hope it is, and I think it should be. Always happened with me anyway.

Amazing thank you so much!Yes my Cardiologist said Wednesday (just gone) that I will need a pacemaker, since phone call I've only received that letter nothing more. So I'm presuming he's arranging another 24 hr ECG before another appointment with him. I'm seeing a different consultant tomorrow so I'm going to see if they can shine any light as I'm also 25 weeks pregnant 🙈 x

My congratulations and my sympathies, not ideal to have treatment for anything whilst pregnant but I can assure you that the Pacemaker implant is done under local anaesthesia and IV pain relief and is normally a day procedure and takes a hour/2 at the most.

I’ve had a resynchronisation pacemaker for 4 years now and it was the best thing I ever did to improve my QOL.

Aww thank you so much each comment is easing my mind more and more so I really appreciate it. I think it's because I've had diagnosis and just been left to wonder myself until appointments come through for everything, so anxiety is through the roof 🙈 x

Just to add that Wenkebach is just another name for Type 1 AV block. I have a Type 2 AV block that I have a pacemaker for and it really has changed my life.

Arr right ok thanknyou for that! It's just he mentioned both 1st degree AV block and Wenkebach in the same sentence as if 2 different diagnosis - miss communication most probably 🙈 x

Crikey that's like a foreign language. I know they write these reports for other doctors but for patients they could at least explain them in plain English. Ask your GP for a translation. I had to do that with my discharge letter. Sorry I can't help.
