Meds for Patient with HF with reduced Ejection fraction ( HF r EF )

Hello saw the Heart Nurse today first time in 18months she wants to introduce Forxiga ( dapagliflozin ) into the mix !! I currently take 7.5mg Bisoprolol one a day, Eplerenone 25mg one every other day, Omeprazole 20mg one a day, Entresto 49/51mg twice a day, Aspirin 75mg one a day, Pravastatin 10mg one a day.

My Question is anyone else on this mix and is it a common mix ? my EF is 40 % and I mange quite well on this mixture Just wondering if the Dapagliflozin being added to the mix will make a difference !!!

My EF is 20%, and I'm just on Bisoprolol, entresto and epleronone. I'm coping reasonably well on this mix.

My EF 30-35 and on Forxiga, Bisoprolol, Epleronone, Aspirin, Entresto, Lansopazole, Ticagrelor, Atorvastatin & Dapagliflozin. Going OK so far but check for signs of Thrush with Dapagliflozin - I keep some Canesten cream handy!

Hi Prada. I have HFrEF, and my cardiologist put me on the waiting list for the Entresto Clinic in January, when an echo showed EF38%. I've no idea if I'm still on the waiting list, but in April another echo showed EF46% (with no changes to meds - I suspect one of the echoes was wrong!), so Entresto is probably off the table. However, in that 3 months I had tipped over into T2 diabetes (Hba1c 50), so the cardiologist wrote to my GP to get me on Dapa. GPs are hopeless, so I didn't get the Dapa until the beginning of July. Unfortunately, I had to give up on it after 15 days because of chronic back pain, which I still have 2 months later (feels like nerve pain rather than muscle, but back pain is a common side effect). GP said that as I'm only just into T2D, she'll do a blood test in December to see if anything else needs to be done. I'm also playing with my statins at the moment, hence GP not wanting to rush into any other drugs or blood tests.

Thanks for the reply Alison. I am on mid dose of Entresto It has been deemed I couldn't tolerate Highest dose hence why Dapagliflozin is being introduced. I do have a decent ef of 40% it has been down to 27% to be honest I am no different at40% than I was at 27 % I think that is may be why in Heart Failure the professionals look at NYHA stage rather than ef numbers !!

I do meet the criteria for a CRT pacemaker but exhausting meds still further before going down that route !!

Hi Prada. My Ejection Fraction had gone down, to between 36 % and 40%, so my cardiologist put me on highest dose of Entresto twice daily, plus Dapagliflozin 10mg. I also take 10mg bisoprolol, 75mg aspirin and 20mg Atorvastatin, They discovered that Dapagliflozin can help remodel the heart, in heart failure patients and therefore introduced this into the mix of drugs. Also Entresto which really is the main drug, has been found to help greatly in some people with lower Ejection Fraction. I haven't found any side effects with either Entresto or Dapagliflozin up to now, however I've only been on them about 6 weeks. Take care.
