Dad has HF EF30%, is BP too low?

Hi guys,

This one is for my dad rather than me. He had a mild HA last week and was diagnosed with heart failure, EF 30%. On a cocktail of new drugs including Entresto and bisoprolol. His BO today is really low, 82/55 and 73/49. Is this to be expected with the new drugs or should I be worried. Called doctor and waiting for a call back but wondered if anyone had a similar experience? HF is all new to me.

I would say it’s low, especially if he feels light headed, normal range = 90-119 / 60-79.

Dr probably needs to tweak his meds.

Thanks Gaz. Doc says he will speak to cardio and for us to monitor BP for 4 days. He’s not feeling lightheaded or anything.

Good, just make sure he is drinking enough water

I would get it checked out Hypotension is equally as damaging as Hypertension is not treated/managed. Depending on age 120/80 BP is recommended up to 65 years of age Iow blood pressure would not pump enough blood to the organs Personally I would make appoint with GP.

Thanks, Spoke to GP and he is going to speak to cardiologist. Dad is in his 80’s.

It depends. If he feels okay with no light headed then wait a few days till he get used to the new medication. If he feels daisy and weak, then call the doctor and most likely will change the entresto dosage or try other medication.

Just in the ‘normal’ range, fingers crossed it settles at a good level.

I am on Entresto, as well as other drugs, and my blood pressure dropped to 96/46 and I felt dizzy, so rang the HF nurse. She noted it, but said it would improve if I rested. Sure enough it came up again . On the mid level now , and waiting for blood test results, but feel much better now I am on Entresto. Wishing your dad all the very best .

Hi. Good wishes to your dad. I also take Entresto (middle range) and was warned that it would lower my BP (as does bisoprolol and Isosorbide mononitrate) and to monitor daily and see if it improves. I;m glad to say that it has, now within normal range again. I drink a lot of water (but watch for fluid retention) and walk around as normal when BP low and it does come up, but take advice about that before doing it. I would be loathe to come off Entresto as I find it to be a really helpful drug - but it needs monitoring.

Thanks Frances. Monitoring BP 4 times a day as instructed by GP and yesterday and this morning it was normal - phew. I really don’t want him to come off the entresto which seems to be touted as a wonder drug. Thanks for your insights, it’s good to know others have had a similar experience.
