Sorry to hear about the worsening ataxia!
I have purchased a few things in the past year and claimed VAT relief/exemption, my experiences have been either ticking a box on a website (if purchasing online) or signing a paper form (if buying in-person) - both cases involve self declaration that you are entitled to it.
As I understand it, the company you're buying from simply add the form to their financial records so when they submit their quarterly VAT returns, HMRC are satisfied (so don't feel awkward if you're dealing with a small company that hasn't dealt with VAT relief before, I'm sure there's help online to guide them through it).

find a building company who specialise in wet rooms for disabled people. They would have knowledge of the needs of people with mobility issues. I had mine done by First Class Builders who have a showroom, maybe there is one in your area or maybe a internet search could uncover one.
A firm who specialise in wet rooms for disabled people would already know about all the forms to complete for getting the vat off.
Personally having tried disabled wet rooms in hotels I prefer my bath lift. However the council did offer to get it done. Apply online for adult care in our area. Good luck 👍 💓
