NHS and local authority partners across Devon and Cornwall are working to increase flu vaccination uptake in eligible groups.
Around 50% of people with an at risk health condition already take advantage of their free flu vaccination and Devon & Cornwall NHS are trying to find out what could increase the numbers even further to better protect people this winter.
They would be grateful if you would respond to this survey IF you live in Devon or Cornwall AND have one of these conditions:
•chronic (long-term) respiratory disease, such as severe asthma, chronic obstructive pulmonary disease (COPD) or bronchitis
•chronic heart disease, such as heart failure
•chronic kidney disease
•chronic liver disease
•chronic neurological disease, such as Parkinson’s disease, motor neurone disease and stroke
•learning disability
•diabetes
•splenic dysfunction
•a weakened immune system

This survey will ask you about what influences your decision to have a flu vaccination. A summary of the survey feedback will be available on the website after the survey closes. Comments you provide in this survey may be used anonymously in project reports and to inform future work.
This survey may be completed by a parent or carer on behalf of someone with an at risk health condition.
The survey is open until 31 August 2014.
