CHEMIST MISTAKE

On Monday I rung my pharmacy.

I asked if they had 3 containers Synthroid 50mgs. I fekt that it. was the 100s and 25mg Synthroid giving me a tiny hard rash and I was going to try all 50s.

The Dr sent the prescription straight to Chemist.

I picked it up as it was ready 3 days later.

I looked in the bag and saw 3. I read the label of each yes 50s.

But this morning I meeded to open first.

Although marked 90 - there were 30 in the first container, seal foil broen, mot in a Synthroid sealed and marked bottom. Yes white.

So itsSaturday and I will need to take them back.

There are 90 synthroid pills in the kiddi-lock square container with condenstion pallet inside and clearly marked 50.

Do you check your meds thoroughly?

Usually I get 100s, and 25s sealed holding 90 in each for 3 months supply.

Hi Joy

It certainly pays to thoroughly check the medication we receive from the pharmacy and your experience is a good reminder to us all.

Thanks for this important reminder. At one point my pharmacy gave me a double dose of BP medication . I could tell it was different so I went back. The pharmacist was horrified. He had a new assistant and somehow it had slipped through.

It's a good job you were on the ball or that could have been very serious.

Hi

Well I have just read the script.

I see there is no 2 repeats.

I'm not sure why Dr wrote "Patient wants 3 containers!

It would be 3 x Synthroid 50s containers so he didn't need to write that. I was explaining..

So what may have had to happen....

90 tabs. = 1 x Synthroid 50

2 repeats. Meaning 90 tabs (in each sealed 50s container square) = 1 x Synthroid 50. X 2 left to go.

But if he had put

3 x tabs Synthroid 50s daily.

3 months supply

I would have got 3 x Synthroid 50s. = 3 x 90tabs in each sealed container.

There are no repeats on the foreign containers. So 30 in each container = 90. only x 3 makes 3 months supply.

Usually I get Synthroid 100 and Synthroid 25

90 in each so 2 sealed containers. Not had a problem with that regime of script.

I think that putting the "patient wants 3 containers is the problem and the 2 repeats as 3 containers sealed 50 would equate to 3 months supply.

Oh dear.

Complicated is the word. But unhappy that this first supply 90 Synthroid had its foil broken, no easy to read 50 and no expiry date. And I never totally break the seal so to help a slow flow of pills to get out.

Yes, the broken foil is concerning.

Hi

Reread the script it is the problem of 1 x tab daily on three containers!

I wrote to my Dr and said please put 3 tabs daily but he put 125mg so awaiting me is 2 complete containers and the other half full. Yet to see it but all in 50s container.
