Xarelto and nosebleeds

I changed from warfarin to xarelto in January and a couple of weeks ago I started random nosebleeds. Only a few, probably 4 too date, but 4 more than I have ever had in my life! I will contact GP on Tuesday, but thought would ask if anyone has any ideas? Thank you 😊

I was put on xarelto in August, and so far have had no problems, but one of my friends had several quite bad nosebleeds at first, but they just seemed to fade away. Her doctor told her he would put her on another anticoagulant if it didn’t stop but it has, and she is fine now. Hope this helps.

Thank you, that is good to know. Just seemed strange they didn’t start till I had been taking it for almost 2 months.

I had breakthrough bleeding on Pradaxa, but have had no problems on Xarelto whatsoever, however, you may be developing some seasonal allergies/nasal dryness while you sleep, etc which can all lead to nosebleeds. I hope you get a handle on it!

I am still on Warfarin but had the most horrendous nose bleed lasting 15-20 minutes last night at about 03:15.

They always happen when I go to the loo.

I wonder if it has anything to do with blood pressure change when I get up.

Hi there Tilly,

Are you by any chance on a beta blocker ...... thinking Bisoprolol, and taking it in the morning.

The reason for asking is that back in Jan 2010 I was diagnosed with AF and was put on Warfarin and 5 mg Bisoprolol. Bisoprolol to be taken in the morning. All well at first, then I started getting quite violent and very random nose bleeds. I initially thought of blaming Warfarin, but for some reason ( can't remember now) I thought about Bisoprolol. Saw my GP who some weeks later changed my Bisoprolol to take at night.

Nosebleeds stopped and I have never had one since. I am still on Warfarin and normally I am constantly in range. I am still on Bisoprolol and in mid January 2021 my dose was increased to 7.5 mg. No problems at all.

Just a weird thought. Good luck.

Hi John, I have been on bisoprolol for about 6 years with no problems - I take 2.5mg in the morning. I used to take warfarin in the morning, until I changed to Xarelto, which I take at teatime. I wonder if it’s worth changing the bisoprolol to teatime.... I will have a think about it. Thank you, Penny

I have always taken my Warfarin at night. So - I was taking Warfarin at night and ( originally) Bisoprolol in morning = bloody noses late morning. My nose bleeds were always around midday ish. sometimes earlier sometimes later.

So then on the advice of my GP I switched my Bisoprolol to the same time as my Warfarin, about 8 pm ish and for me ....... job done.

I regard Bisoprolol as "my dirty drug" - for no other reason than it needs handling with the utmost respect. I was originally prescribed it for heart rate control, but, I understand it has magic properties that make it suitable for blood pressure control too.

Look, I'm not a medical person, so, why not discuss changing your Bisoprolol to night time with your Xarelto with your GP just to make sure no harm can arise. Hope it works for you.

I started off taking bisoprolol in the evening, but it really didn’t sit well, so I changed to taking in the morning and it felt better (or as much as could) Then, because I kept forgetting my warfarin at teatime, the final straw was forgetting to take it at all one day!, I changed that to the morning too. I researched the internet inside out as to why warfarin should be taken at 6pm, asked all sorts of people. All I came up with was that it helped the medical profession if it was generally taken around the same time. I couldn’t find a medical reason 😂. So I started both with breakfast, problem solved. My next step, after swopping warfarin for a DOAC, is to look at reducing bisoprolol or even changing it. Since starting these meds, I have become so lethargic, tired all the time, and the aches, pains and stiffness are unbelievable. I can only take paracetamol for pain relief, which doesn’t do much, as I am allergic to codeine ! Wasn’t till recently I realised I didn’t feel like this before starting the bisoprolol. Not long before I had completed a power walk marathon, whereas now I can barely walk 500yards without feeling exhausted. I am only 64. I understand it was prescribed for heart rate as well as bp. Joys of AF 😂🥰

Here is another interesting tidbit - I find if I come off gluten, I rarely get af episodes ........

Why not discuss reducing your dose of Bisoprolol with your GP. If you are getting few afib attacks what is it for? Or you could ask about taking it as a PIP to reduce heart rate during an attack. Some people just do not tolerate beta blockers. I felt like a zombie on 2.5 mg Bisoprolol.

Hi Penny,

I think AF and its associated medication is like a voyage with Captain Cook - a voyage of discovery and that discovery process is something very personal to us all. And very different for all of us. For me Bisoprolol is no problem, i.e. I'm now 76 and still drive buses 3 days a week and can still jump through all the medical hoops DVLA put before me prior to renewing my PCV drivers licence.

Yes, I do have my heart chucking a "wobbly" every now and then. At no point have I had to rush off to A & E, did have paramedics out in January 2021 when my ECG device showed some weird lines, otherwise, I may have a wobbly 2 or 3 times a year - BUT - not every year.

However - you mention diet. Way back in 2011 I realised a connection between my gut and my heart ( vagal nerve) and consulted a Nutritionist who advised me on diet. So from that point I went gluten free, wheat free and oats free ......... and since those early days with the Nutritionist I have expanded my diet to include a much wider range of stuff - the cumulative effect has been to help minimise my AF. I would never consider myself cured. However, it certainly has brought a massive degree of control over my AF and my life is now pretty well normal. Brill.

Maybe it could be time to do what Auriculaire suggests re the Bisoprolol.

I'd be interested to know what you do that sorts out the nosebleeds.

Hi,I use a handheld device, a Healforce PC-80B. Nothing mind shattering. I don't understand ECG graphs so couldn't see the point of spending shedloads on a Kardia.

If you Google my device, then Google Omron and/or Bauer you'll see a range of stuff. A range of prices too🙂.

My philosophy is simple, I use a variety of devices for different diagnostics and then tend to ignore the values that a device produces but look at trend lines, short term trend and longer term. Similarly with an ECG if the device produces a brief advice saying, say, irregular heartbeat or slow heart beat with irregular heartbeat then if I lasts for any time then I would see my GP. Hope that helps.

There are nosebleeds and there are nosebleeds. I've not had a nosebleed that runs, but I have frequently woken in the morning to find blood clots in my nostrils, or else I get red looking tissues all day without any blood actually running. I can then go several days without any sign of it. I did meet one chap on Warfarin who had a nosebleed so bad even the medical centre couldn't get it to stop. I take Apixaban.

Nose bleeds happen even if you have think old blood like mine. If it didn't stop bleeding that would be bad. But its more an issue of your blood vessels being weak in your nose and if they dry out they can burst. But I don't think it matters about the blood inside them. Vitamin c helps keep the walls of your blood vessels strong... And making sure youre hudrated.... Turning off the central heating or adding a humidifier. It could.just be a coincidence. And presumably you went on antigoags because you havent the best of health so that could affect your nose too. My kid who should be healthy gets nose bleeds a lot but I know why he gets them...picly picky ...my mom gets them too same reason...I only get them in the desert....... But if you are not well enough to be on blood thinners you probably need some help with strengthening your capillaries too. .... Or it could be that your blood has been sp think for so long it hssnt reached into those small capillaries and now it flows it can get in there and your nose isn't used to the nice flowery blood.... Vitamin c zinc collegen ...humidifier....unless the nose bleed doesn't stop they're not harmful .

Hello I've been on xarelto for about 3 years and have never had any problems it's sometimes makes me wonder if it's actually doing anything.

I was on Xarelto for a couple of years with no problem and then suddenly I started getting nose bleeds. I would wake up in the middle of the night with blood coming down from the back of my nose and into my mouth - ugh, horrible. Sometimes it took at least 20 minutes to stop. This lasted for many weeks and eventually my GP changed my medication to Apixaban which has to be taken twice daily, as opposed to Xarelto which is only taken once. So far no problems thankfully.
