Sadly I have to have a molar removed. My EP has said provided INR is 2, I will bleed a little more but not much. My question is what about the use of adrenalin in the injection, I have a pacemaker.

You can ask for adrenaline free local, your dentist may well be using it anyway. When I was dental nursing anyone with a heart condition automatically had adrenaline free injections.
I would definitely ask them to use adrenaline free local anaesthetic.
I had some really bad experiences before I realised that it would cause my heart to pound and race.

I agree that non-adrenaline anaesthetic is a good way to go My dentist was concerned it might not anaesthetise as much as I needed, but it was absolutely fine.
&There’s people on here who have had several extractions with no problem.

It is not adrenalin - it's called epinephrine. Most dentists have non-epinephrine syringes. And any AFibbers do not want epinephrine in their system!
I have non adrenaline local anaesthetic ( also known as no epinephrine). It seems to work just as well. Our dentist says AF is not on their list of diseases which require no adrenaline so as well to ask to make sure. My husband has had several extractions while on warfarin, he just gets an INR the day before and again dentist says no worries as long as under4 !!!
PS if one is anxious about dental treatment there will be an increase in adrenalin in the system anyway as it is the " fight or flee" hormone.
