Apixaban,hospital and dentists

Hi I posted recently regarding a tooth root abcess . I have an appointment at our local dental hospital ...20 th may (!) They will cut through the gum ,remove the rubbish and fill the top if the tooth root. It's a front tooth.

Over the bank holiday weekend,I gradually went downhill. Had had one lot of antibiotics,which finished the week before. By yesterday morning,I was red hot ,shivering and in agony.

Called the local dental hospital to see if I could be seen earlier...no. called my dentist,brought me in to surgery,took one look at me and packed me off to A and E at our local teaching hospital as we have no dental dept nearby.

Tested for sepsis,thankfully not but severe infection. Had fancy x-rays,was seem by Maxofacial person,told abcess is self draining onto my sinuses,so they will stick to original plan,see me on 20 May....mega strength antibiotics meanwhile and co codamol.

My EP told me to stop Apixaban 2 days before,and that any local anaesthetic should be ok! The Chap yesterday said no ,not necessary to stop Apixaban,and not to have an adrenalin based one! Today,had to pop into the dental hospital,and they said the same !

Confused ....not really but it is a bit worrying when conflicting advice is given!

I thought everybody here knew not to allow adrenalin based local anaesthetics as they can trigger AF in some people if they have it. In my view the dentists should know as this is what they are doing all day long. My dentist never bothers about me being on warfarin.

What a horrible misery for you - hopefully, the antibiotic treatment will work quickly. I’ve had an emergency ‘smash and grab’ raid on a lower wisdom tooth hours after I’d taken my Apixaban dose, had adrenaline free injections as usual to have the roots cut out without any problems. (Other than mind blowing jaw ache that is 😩).

It doesn’t surprise me that you’ve had conflicting advice but your EP is surprising!! The problem is that two different medical disciplines are involved and see differing facets of the same situation. Perhaps MaxFac would be most knowledgeable from their experiences across many patients.

Thank you Finvola...yes misery and surprising that Professor O didnt seem to know the ins and outs.....!!! Looking forward to the day of doom.....shall lie bqck and think of England..or Wa!es as im half Welsh......xxxx

I have just had a course of antibiotics for a chest infection only to develop a very bad water infection , Dear me if it is not one thing it is another.

Sorry your abscess is giving you so many problems, I totally understand, I developed a high temperature and the rigours with mine and really worried my daughter as my husband was away.

My dentist says no problem with any treatment if INR is under 3 and sounds totally confident. Of course Apixaban works differently but I think that many dentists are up-to-date and in the know about treating patients on anti-coagulants who might bleed. I think in your situation I would go with the dental hospital's advice!

Please take extra care when taking all these strong antibiotics, and replace the gut flora that will have been destroyed. There are lots of links explaining how to do this.
