The lower your T level, ideally below 20 ng/dl, the better your outcome. I'm sorry you're suffering the side effects of low T -- it sucks, but that's what it does.

11.5 is my T level I get headaches from the Orgovyx and was thinking about reducing my dose to lower side effects and wonder if anybody has done that and kept T low thanks

I for one take 1/10 of Bicalutamide dose and didn't ask anyone. In engineering we say that a test is worth more than the combined opinions of 10 experts. Reduce the dose gradually while having frequent labs. The good thing about Orgovyx is that it has a short half life and as such it will not take you long until you find your Minimum Effective Dose.

How frequently should he check T and PSA. Should he do it himself if doctor won't do it frequently enough?

Doctor will probably freak out upon hearing such a scheme. Too cumbersome for the typical doctor. I am not prescribing but, if it were me I would started with 3/4 dose for 15 days and tested. If no substantial increase observed I would kept the same dose (3/4) and retested in a month. 1.5 months later (3 mo from start), provided all is normal then I would have gone to 1,/2 dose following same monitoring schedule. But, I am just the odd one out here, so don't take me seriously. One thing to check beforehand is whether the tablets have slow release formulation or encasement to pass through the stomach and be absorbed when into the intestines. This could be a valid reason for the advisory not to split, chew, crush etc that has been posted..

thankyou once my testicles are removed i plan on taking a low dose of bicalutimide asyou have done for an androgen blocker for any testosterone produced elsewhere I think bicalutimide should work fine to block the small amount of T and hope i can tolerate it I will start at 50 mg ?? how long have you taken it

I don't take Orgovyx but I have cut in half every hormone suppressing drug I have been on with no loss of efficacy and much reduced side effects including Zytiga and Apalutamide.

good to hear My theory is that the drug manuf. use the highest dose possible for their trials as they want the drug to succeed so they get the gov t approvals if they used a smaller dose they might have a slightly less success rate But the lesser dose would probably work for most of us Thankyou

Agree. It took me a while to have the confidence to cut the dose but my Onco was supportive when I showed that high blood pressure was mitigated by reduced dosages. And then blood work showed no change in PSA.

thanks for that info

From Goolia, Fran and Olie.....

What is Orgovyx?

Orgovyx tablet (relugolix) is a prescription medicine used to treat advanced prostate cancer (cancer that begins in the prostate) in adult men. Orgovyx is a once-daily tablet that quickly and effectively decreases testosterone (a male hormone) levels. This helps to slow or stop the spread of prostate cancer cells that need testosterone to grow.

Relugolix belongs to a class of medications called gonadotropin-releasing hormone (GnRH) receptor antagonists.

How does Orgovyx work?

Orgovyx works (mechanism of action) by binding and blocking GnRH receptors in the pituitary gland. Blocking GnRH receptors reduces the production of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which in turn lowers testosterone levels in the body. Since most prostate cancer cells depend on testosterone to grow, Orgovyx helps slow or stop cancer cell growth by effectively reducing testosterone production.

How well does Orgovyx work?

Orgovyx was fast and effective at suppressing testosterone in advanced prostate cancer in the Orgovyx HERO clinical trial (NCT03085095)

Orgovyx effective testosterone suppression:

96.7% of Orgovyx patients maintained castrate levels (<50 ng/dL) from day 29 through to 48 weeks vs. 88.8% with leuprolide.

Faster action:

Day 4: 56% of Orgovyx patients reached castrate levels vs. 0% with leuprolide.

Day 15: 99% of Orgovyx patients achieved suppression vs. 12% with leuprolide.

Good Luck, Good Health and Good Humor.

yes but they don t want to be sued and therefore will not recommend less than was approved even though other patients have lowered it with success You see to get a drug approved the manufacturer usus the highest dose to ensure the drug works and then they get the approval and $$ They could probably have got the approval on a lesser dose but why take the chance They want the approval. The approved amount then becomes the standard of care for doctors to follow even though some patients could succeed on a lesser amount But there is a risk which only the patient should take depending on why rhey can t take a full recommended dose. thanks for your thoughts
