Orgovyx hot flashes

I've been on Orgovyx for about 8 days. Hot flashes happen about 3 times a day. Will the hot flashes eventually fade as tone goes by? Is there any way to help with them ?

If you can get a scrip for Veozah (it's only approved for post-menstrual women so far), that is the best, IMO. Other solutions are aural acupuncture, estrogen patches (±tamoxifen gel), Megace, venlafaxine, or oxybutynin.

I tried all the usual including Acupuncture (which worked for a while until I took time off) to no avail. Finally started Veozah which has been amazing. Insurance won't cover it for men, I went through 3 appeals. Many men can have their wife get a script, mine had a conflict due to other meds. I paid out of pocket - $573/month - it was worth it as my flashes were epic - every 2-3 hours. Paying $18/day to get rid of the flashes has been worth it.

Mine have been non existent since I started an aggressive workout program

What kind of aggressive workout program do you do? Has the workout helped with other side effects of ADT too?

I will say I had the same results using exercise. I do 45 to 60 minutes of cardio. Treadmill and walk, hike.

I do 30 minutes of resistance. Bench, curls, etc. light weights.

20 minutes of stretching, balance and sit-ups.

I do this 6 days a week.

Exercise is most important and really helps in this battle living with cancer and help minimizing the side effects of ADT.

I'm back on ADT , Orgovyx, after a 6 month ADT vacation. Lots of muscle gain during the vacation by working out with cardio and resistance training. It's an uphill battle now that I'm back on ADT keeping my gains but feeling confident that I will push through the hump .

My experience with Orgovyx is the hot flashes get worse with time, unfortunately. I was on it for 10.5 months, off it now for 1.5 months and even now I get one every 2 hours, even through the night. 6 months ago they were about every 3 hours. I hope you have a different outcome.

I experienced the same: hot flashes that got worse over time.

The good news is that when I finished Orogovxy after 36 months on the drug, the hot flashes gradually stopped.

Estradiol gel from you local compounding pharmacy can be had for $1 per day, and is very effective. Ask your PCP to prescribe it. Apply to upper buttocks or hips, or for higher absorption, to the jaw line or scrotal skin. Application to the forearm is not recommended, because of low absorption rates.

Only about 100-120 pg/ml of serum E2 is needed to completely eliminate hot flashes (in post menopausal women), and the same effect is assumed to apply to men. That is 2-3 times more than the normal range of estradiol in men, which correlates to a low-to-medium dose of E2.

The high dose used by the PATCH study (which reached, on average, serum E2 level = 230 pg/ml) used 3-4, simultaneous, large estradiol patches.

You could likely eliminate your hot flashes with only 1 large estradiol patch at a time, changed once or twice weekly. Gynecomastia is not expected to be a big problem at this low dose (however, some changes may be seen eventually).

I have been on Orgovyx for 15 months, the hot flashes continue, but don't bother me much. I do an hour of almost daily exercises so maybe that reduces their intensity.

If you only have three hot flashes a day, count yourself lucky. I have been on Orgovyx off and on for over three years....my view is the hot flashes are just an annoyance while enjoying quality of life while keeping ProstateCa in check for now.

21 months in and they have not faded for me.

I appreciate all the postings here. I asked in another post about whether my flowmax was causing my increased hot flashes, and I guess the answer is it isn't. It's probably just the Orgovyx. I feel like they're increasing at the 13 months mark of taking it. Annoying. Any other suggestions for relief?

Do you exercise? I do exercise regularly but only about 2 weeks in on Orgovyx

Every day, alternating walking (2 miles) with gym days (machines). Mainly for the muscle loss during ADT.

Although I exercise regularly the hot flashes still interfere with sleep

try 300 mg gabapentin. Start with 100 and increase. Worked for me on hot flashes and some other aches

When starting Orgovyx, how long before side effects are felt, and what do they consist of? Is it just hot flashes and ED? What exactly are hot flashes?

rosenjpj profile image
rosenjpj in reply toHavanablues

Hot flashes = feeling overwhelmingly hot and sweaty. They are usually short lived (half hour) but hot flashes can come on several times a day. Other sexual side effects include loss of libido (sexual desire) and difficulty in reaching orgasm. Also, weight gain and associated rise in blood sugar. With extended use there can loss of bone density (susceptability to fracture and breaks). Some experience cardiac symptoms too. The following from ChatGBT

Common Side Effects:

Hot Flashes – One of the most frequent side effects, similar to other ADT medications.

Fatigue – Often reported, especially during the first few months of treatment.

Weight Gain & Increased Body Fat – Metabolic changes may cause fat accumulation, particularly around the abdomen.

Loss of Muscle Mass – Due to reduced testosterone levels.

Decreased Libido & Erectile Dysfunction – Common with testosterone suppression.

Gynecomastia (Breast Swelling/Tenderness) – Rare but possible.

Mood Changes & Depression – Some men experience emotional changes, including irritability or mood swings.

Cognitive Effects ("Brain Fog") – Some report memory or concentration difficulties.

Metabolic & Cardiovascular Effects:

Increased Blood Sugar & Risk of Diabetes – ADT can affect glucose metabolism.

Elevated Cholesterol & Lipid Changes – Can contribute to long-term cardiovascular risk.

Possible Risk of Heart Disease – Some ADT therapies may increase cardiovascular risk, but Orgovyx has been suggested to have a lower risk compared to injectables like Lupron.

Bone Health:

Bone Density Loss (Osteopenia/Osteoporosis) – Long-term ADT can lead to weaker bones, increasing fracture risk.

Did you use any other ADT like Lupron before Orgovyx and did you experience hot flashes then as well?

Slugger61 profile image
Slugger61 in reply toVman1

Yes I was on the triple..Lupron shot, abiraterone and Prednisone. I did experience hot flashes during first 3 months on a 6 month Lupron injection. A couple times a day I felt them. This Orgovyx feels like it delivers a stronger hot flash about 3 times a day. Hoping they will fade. But hey if it keeps cancer in check and away then the hot flashes I can handle. I can tell I am sure glad I'm off the Prednisone..That med made my skin crawl in my head

Thanks. I had some hot flashes in the beginning with my six month Eligard shot. Then they completely went away. Will be starting Orgovyx. It will interesting to see if I get hot flashes.
