After 40 day EBERT radiation AND 2 LOTS OF ZOLADEX in 2021...PSA was going up very slowly but In 2024 went from 1.5 to 2.5 in 3 months....and my psma pet scan showed 5 very small spots 3 in pelvic nodes and one in lower outside bowe area.....But in my 3 yrs after initial treatments .. my specialist kept me on watch and wait but with the sudden PSA 2.5 and testosterone 10 increase Dr Tan has suggested that I go on a trial..with Darolutamide...and a course of ADT therapy. And it's a randomised clinical trial at the ROYAL ADELAIDE HOSPITAL... Where 50% will get a placebo and the other will get the real tablets and also they will do multiple psma pet scans throughout the 2 year trial....or would I be better off getting the real tablets and ADT treatments outside the trial knowing I'm getting the real tablets instead of the sugar pill. I'm 76 and was ist diagnosed as mentioned in my Bio around 4 yrs ago....thankyou for any advice that may help me going forward cheers Richard....(User name millythefox).👍

Could you please provide a link to the clinical trial on clinicaltrial.gov?

Can you get your pelvic lymph nodes irradiated outside of the area that was al ready irradiated?

I suggest you discuss that with your radiation oncologist in addition to discuss with your oncologist following the EMBARK or PRESTO protocol:

Thankyou for your advice appreciate all your help ...I'm wondering if to get my testosterone down permanently is it advisable to remove testicles as my first round of 40 day EBRT radiation treatment with 2 rounds of ADT Zolodex had reduced my PSA below 0.1 from 138 And testosterone from 15 down below 1.....and for 3 years it barely moved until b4 Xmas PSA went from 1.5 to 2.5 in 3 months Dr Tan called it a Biochemical relapse hence want me to go on this trial which all patients will get the ADT injection every 12 weeks but only half will get the Daralutamide pills and half the placebo.....I am still trying to decide what is best going forward and that's where I'm at...bearing in mind when I was ist diagnose just over 4 yrs ago I had iliac lymph node and sidewall lymph node and seminal vesicle involvement to start with and a Gleason 4+3 with an undesirable outcome...also diagnosed with type 2 diabetes and put on Metformin 1000 as well....also take CBD cannabinoid oil under prescription also .I need to give an answer to dr Tan tomorrow when he calls me for my decision..so not much time left.
