hello all- my father went to a urologist for reoccurring urinary track infections so totally unrelated to his prostate cancer. While he was there, the urologist told him that he should get genetic testing done for his prostate cancer in order to help determine next steps. He is in a situation right now where his PSA is rising (doubling about every three months. Current PSA is around 2.0) after about six years on xtandi. We are going back to his oncologist next month and most likely a PSMA scan will be ordered- which will be his first. He’s only had CT scans and bone scans up to this point., but I’m wondering if any sort of genetic testing could also help determine next steps.

Most definitely, I tested positive for BRCA2 and my treatment going forward is different than those without the genetic defect

Consider the PRESIDE protocol:

If the PET scan shows any metastases, consider a biopsy and histology, IHC and genomics.

TA, my scan showed 3 pelvic lymph node metastases, for which I received radiation. Should I inquire about these things you mention, including genomics, which I think is genetic treating? Thanks.

You no longer have anything to biopsy.

I would ask for germline (spit or oral swab) and somatic if he has any tissue sample left from his biopsy. They do degrade with time.

They can also do a liquid biopsy. It can tell what is circulating in his blood stream. They all go under different brand names.

Also, the PROMISE trial offers all men diagnosed with prostate cancer a free germline genetic test. It’s fast and gets tracked in a research database for discoveries. They mail the kit to your house and even report the results to you and your doctor. If I recall it took less than a month.
