on what schedule are most folks taking their abiraterone……I thought I had it right initially but now a pharmacist is telling me I should wait 2 hours after taking Abi before I eat or take anything else….. thought 2 hours/1 hour was the general rule…….i was taking Abi upon awakening…..then at least an hour later the rest of my meds with a small amount of food……

that’s what I was led to believe initially….and why I am asking the question…..pharmacist at bio-plus pharmacy says otherwise….. I’m going to stay with the 2 and 1 protocol.
From the beginning, I have taken the Abi/Pred right after a get up in the morning, then by the time I am done in the shower and shave and such, then make coffee, an hour has passed.
No real issues other than the regular side effects.
Abi in its regular form is working well for me and I don’t have to pay for it right now……so I’m not going to make any changes…..but……half the dosage of Abi and pred sounds appealing.
take it 2 hours before eating prednisone tablet with food but have found must take a large amount of water with tablet or can affect stomach or liver 3x times I had very bad stomach upsets
On holiday now but when taking AA preferred 25 % of regular dose with healthy lunch. Amount of prednisone was 100 % of regular dose (because the AA in blood was the same with this approach)Cost was not a factor. Primary motive was to avoid some discomfort with breakfast when taking it at 0500 or 0600 hrs.
I tried taking 1000 mg on an empty stomach but it caused too much nausea so I tried 250 mg with breakfast. It was not enough. Now I take 500 mg with a meal and it works fine. My meal is 300-400 calories and 8-10 grams fat.
My pharmacist explained that since Abbie can make you tired after you take it, why wouldn't you take it just before you go to bed. Made sense to me so I stop eating around 8:30PM and take the pills around 11PM. If I want to go to bed earlier, I just stop eating earlier.
My husband's pharmacist told him to take 2 hrs after a meal and he also recommended to take them at night because one of the side effects are fatigue and that is why hubby takes it before bed around 10 pm.
I was under the impression that tiredness is a overall side effect that is independent of the dosing schedule but everyone is different so if you get it after a dose nighttime would be a good idea.
I have an associate in Toronto who is taking it . He has been on Active Surveillance for 5 plus years . It is not physician prescribed , as you indicated . He maintains it works .

there has been some confusion on this point, and to make things worse - the label on some ABI bottles also states it incorrectly. It's 1 hour before and two hours after.
And if you think about it, that makes sense. You're trying to not digest both the ABI and food at same time. If you wait for 2 hrs after eating, that minimizes the risk.
The ABI itself is metabolized more quickly than food, so that's why it's only 1 hr before taking the ABI.
So using common sense here is how I remember the protocol.
When he first started in 2020, he’d get up at 5:30, take 4 pills, then eat breakfast at 7:30 plus the prednisone. Some mornings if he woke up at 4:30 am, he would do it then
But right now he’s taking only one tab with low-fat breakfast. It’s a stopgap move, had to stop chemo to do hyperbaric oxygen treatments for the next 2 months
Personally, I wouldn’t worry too much about that. We’re all taking so many meds that trying to figure out optimal timing for eating is a bit extreme. I’m happy just to have my Apple Watch (thank you, Amore) remind me to take the damn pills morning and evening. Look, your meds, over weeks and months, average out over many different conditions in your life, including during travel. Just be sure to take them and while you focus on maintaining your QOL. (My Abi is working well and doesn’t seem to care much about what or when I eat.)
my routine… take Abi 6:30 am, followed by I/2 dropper of CBD @ 7:00, eat breakfast between 7:30 & 8:00, with prednisone after eating. Works well with minimal side effects. Going on 7 years Mpca. And PSA still undetectable. Living life and trusting God. 🙏
72 year old male, Gleason 8 - when I started taking abi in Feb 2024, I was NOT adhering to the 2 hour protocol (no food 2 hours before & after taking daily dosage of abi) - I was experiencing extreme fatigue and had what I would call vertigo several times - starting in July 2024, I was taking my daily abi during my 6 AM visit to the bathroom - this assured 2 hours before AND I never eat before 10 AM - so the protocol was adhered to - since that July change, the fatigue has gotten much better AND no more vertigo - GOOD LUCK!
It was just a few time but very unnerving. I would get up in the morning and practically fall over in taking a few steps. It had nothing to do with heights
