My dad had 2 shots of Pluvicto and each time his PSA went up. His oncologist stopped treatment and said it wasn’t working . While waiting for other options and a mutation test (no mutation), his PSA has dropped 15 points. Could this be a delayed response? Anyone else have a delayed response?

I also agree it is delayed response. What's ALP trend looks like ?

Usually scan before Pluvicto start and scan after 2 Pluvicto injections to understand whether it is working or not

Does he takes other medicine ?

ALP normal.

They have not done scans since February. He started Pluvicto in May/June. I’m guessing Medicare won’t pay for them that often.

The previous increase while using Pluvicto may have been due to cancer cells dying. Suggest a PSMA PET scan to decide.

My husband’s MO, Dr. Oliver Sartor, co-principle investigator in the Pluvitco phase 3 trial, prepared my husband to expect a spike in psa for the first couple of treatments. It’s a very normal and understandable reaction. The radioactive molecule binding to the cancer cells for the first time is like disrupting a contained bees nest so to speak. If you look at it that way, Pluvitco causes quite an upset in cancer cells explaining the initial spike in PSA. I envisioned it as a surprise attack LOL The fact that your father’s psa has now dropped after his first couple of treatments appears to be an indication that Pluvitvo is indeed working! It’s also protocol to have a PSMA pet scan after the 3rd treatment (the half-time show). Why guess when you can actually know for sure? And to answer your other question, yes, this was exactly the response my husband had to his 6 Pluvitco treatments…2 month spike then a continued drop from then on. Please do not settle with your father’s MO opinion. Press to continue Pluvitco unless there is additional concerns or side effects that you haven’t mentioned. I love how you’re advocating for him. He’s lucky to have you. Hoping for the best for him!
