Why Cabazitaxel instead of 2nd Docetaxel?

For people that Docetaxel worked, why do they typically switch you to Cabazitaxel for the 2nd time of chemo instead of repeating the Docetaxel? Thanks.

Because some of the cancer may be docetaxel-resistant. But docetaxel might continue to work if it worked well before.

Good question. I responded really well to docetaxel when diagnosed six years ago. My oncologist suggested that we might try it again once my current abiraterone / ADT regimen starts to fail.

Some prefer Cabazitaxel due to its reported lower side effects, but in many cases, it's not covered by insurance as first option.

When you say that docetaxel "worked," can be you be more specific about what you mean and how that was determined, particularly since you now find additional chemo necessary?

Thanks for the clarification.

Like most of my different treatments, it held the tumors at bay for several months. But unfortunately about 2 months after I stopped Docetaxel, my PSA started rising and tumors increased.

I was treated with Docetaxel again after 5 years. It worked for about 6 treatments and then started failing. I was then switched to Cabazitaxel and it has been working with less side effects. PSA is slowly rising but no progression of disease according to the CT and Bone Scans. Since November last year I have had 7 treatments of Docetaxel and 7 treatments of Cabazitaxel. They want me to do 10 in total. Definitely keeps the pain away. I am waiting for a trial Lu-177.

Thanks for the info. Where are you doing your chemo sessions? And who is your oncologist? And of course, I would like details on your possible trial Lu-177. Do you want to discuss via 'chat' or talk on the phone? Thanks. Oh, with your Cabazitaxel, are you getting dexamethasone, I'm getting that and it's been great for my energy.

The trial is still not confirmed. Waiting for another appointment. BC Cancer in Vancouver. Yes using Dexamethasone every day. I thought it was great for energy but after long time use you have a lot of swelling though out your body caused by the steroids.

Do you have a link to the trial? Also, how long have you been on dexamethasone, I've been on for 2 months and getting swelling in my legs, started to wear compression socks.

No link. Having to push doctors to find another treatment after the chemo fails. I have been on steroids since November. Need to watch your diet.

Thanks. I’m in similar position, I’m running out of options. What do you mean, need to watch your diet?
