Experience/Opinions on: Pembrolizumab + Lenvatinib for NEPC/SCPC

In January I was diagnosed with SCPC with liver metastasis.

My first line of treatment was Platinum based chemo (carboplatin+etoposide). This treatment failed after 3 infusions with radiographic progression on CT Scan.

HEPATOBILIARY: Innumerable bilobar hepatic metastasis have progressed. There are several new lesions and enlargement of the pre-existing lesions. For instance, a 1.5 x 1.1 cm lesion in segment 7 is now 2.7 x 2.5 cm. There is mass effect narrowing the hepatic vessels but the portal and hepatic veins remain patent. No biliary dilation the gallbladder is unremarkable.

For the 2nd time this year, I was given an estimated 90 days to live without effective 2nd line treatment. Also my MO seems to stand behind this estimate since he has registered me with BC Palliative Care Program (6 mo's to live).

At my urging, my MO reached out to US based Clinics in Seattle, Portland and San Francisco with the goal of seeking out clinical trials relevant to my condition.

Within 1 week I have a spot reserved on the PLANE/PC trial contingent on meeting the eligibility requirements. We are in the processing of working thru that now and I will probably have to travel to Oregon in the near future.

I like that it is tailored to each patient's type of small cell PCa.

This study: NCT02861573 has been around for a while and includes at least one arm that combines the same two drugs. It was on my treatment buffet for a bit before deciding to try PT-112 (which didn't work for me, at least for long). It's part of the KEYNOTE series of trials that seem to be funded to look for combinations of drugs with pembrolizumab that are effective. I'd poke around and see if you can find any results.

I've talked to some MO's about the trial. It's not overly encouraging. I'll post a synopsis of the conversations a little later after I collect some more information from the Clinical Resource Coordinator.

I've already decided to start Docetaxel immediately and push CT's as a possible line of treatment into the background.

My first tx once we discovered my NE component was docetaxel + carboplatin, hopefully the docetaxel will get what the carbo didn't!
