Rouge clone radiation on 10/31/23 has PSA from 4.83 on 11/20/23 to 0.85 on 12/18. HOWEVER, PSA up to 1.48 on 1/29/24 then 2.48 on 2/15/24.
Waiting MO response but anticipate PET scan, radiation and/or switch to Xtandi. Was hoping to ride the Zytiga therapy as long as possible but I may be there now.

My MO says "You are describing is what we call oligo progression and the few lesions that might show growth may be what I call Rogue clones and usually what I do is not stop or change the current treatment and just add stereotactic radiation to the areas that showed progression usually 3 lesions or less."
From Goggle:
Oligoprogression is an emerging concept in oncology, denoting a state where after an initially successfully systemic therapy of disseminated metastases, a single/few lesions display further progression.
Ok, just never seen that terminology before!! A number of studies confirm benefit for some men for RT when oligometastatic. GOOD LUCK !!!!!!!!!!!!!!!!!!!!!!!!
Xtandi is not that bad as soon as you learn now to contend with side effects. Highly recommend getting on a good diet and exercise program. Seems biggest side effect is weight control and just getting lazy.
I have been on Zytiga since 8/21. It has been doing it's job in keeping my numerous bone mets at bay with only 2 lesion growth incidents - Oligoprogression. I believe Xtandi, like Zytiga has a useful time limit so I want to delay it's start as long as possible. Seeking comments on treatment plans.
