Does testosterone fluctuate after ADT ends?

My treatment (Radiation/Lupron/Abirateron/Prednisone) ended on May 2. Gradually my testosterone began to come back. In August it was at 3 nmol/L and in Oct 6 it was 11 nmol/L so it is on a upward trend as can be expected.

However I am wondering if there are up and down fluctuations because while my lifestyle is the same from day to day, once in a while a get a full day of increased fatigue but the next day I am fine once more. Could be a virus but if so, it is a short one since it is gone in 24 hours.

The other thing is that on two different occasions around mid-September, I woke up with an erect penis, something that has not happened in years. My libido went up and I was even able to masturbate properly around that time.

But for the past 2-3 weeks, I am once more unable to get an erection and my libido has vanished once again as if I was still on ADT.

So I am wondering if there are some misfires with my testicules or if this fluctuation is typical of recovering from ADT and that I should be expecting these ups and downs for a while?

Testosterone levels vary a lot during the day and day-to-day.

Since you're using a scale I'm not familiar with (nmol/L) I can't comment on your actual numbers, but as  NotDFL commented - T levels can vary a lot day to day. Mine bounces around between about 280ng/dl and 350ng/dl. I suspect when I'm feeling particularly horny it's toward the 300+ area.

I wouldn't be overly concerned. You're only 6 months off ADT - and it can take a year or so for T to recover and reach a sort of steady state. Since you seem to be achieving occasional erections you should be celebrating in some manner..

Yes it does fluctuate quite a bit. Try to always get tested in the mornings.

Thanks for the suggestion. Most of the time, they take my blood at 11h30am.

The peak time for T is about 8 AM. The trough time is about 8PM.

I dont know anything about T fluctuations. But I experience variations in libido and erections even with "zero" testosterone. I did so also before diagnosis as I think most men do. My conclusion is that overall wellbeing and psycological and emotional status is relevant. It is not all about T. Stress and worries kills my libido and my erections as well as lack of T does.

I agree that body and mental health is an important factor where erections are concerned. That is why I specified that my lifestyle is the same from day to day. My days are like groundhog day as far as my health and mind are concerned. That is why my thoughts went to testosterone being the key factor in this case since everything else is as usual.

Yes, I understand and agree. Just wanted to include the wellbeing aspect since I myself tend to work and worrie to much. Not beneficial to relations either.

T levels do fluctuate a lot, but what you’re experiencing is probably just as much the non-linear nature of T recovery post ADT. Energy levels up and down, same with erectile function.

If they continue to trend up great. At some point they will level off. Then hopefully you continue to embrace the slow steady decline of aging. 😀

I tried that once, about a decade ago, it gave me such a migraine that I ended up not having sex. So I'll stay away from Cialis or Viagra :)
