Issue with pain

Hello gents, my father 63yo was diagnosed with PC s4 at the end of 2022. T4N1M1b, PSA=30, G=7 (4+3), more details in profile.

January 2023 started ADT (Zoladex)

June 2023 went to MO with severe pain in his lower back and lumbar area, and started Chemo (Docetaxel).

He just finished his 2nd cycle of Chemo few days ago.

The biggest issue is a severe never-ending pain all around his lower back..

Can IMRT radiation help? Can we do radiation now and dont wait till the end of chemo?

Is there anything else what can help?

Please help me with future steps on this issue.

Chemo may resolve the pain. He should also be on abiraterone. If they don't help, radiation might.

MO suggested to add abiraterone if Docetaxel go well.

Do you think we need to wait with the decision on radiation till the end of chemo?

The chemo+abiraterone may control the pain. It often does.

Radiation has been extremely helpful in reducing pain and shrinking Mets for my husband who is cr and currently on docetaxel and degarelix. Apalutamide and abiraterone both became ineffective.

I suggest your father gets a Lu177 therapy at the Nuclear Medicine Center in Baku, Azerbaijan. Our member RusLand was treated there successfully.

My father had alot of pain in the back and pelvis two years ago and they did a dose of full pelvic radiation and a spot to the back, its been 2 years and he is relieved of that pain, radiation is good for the bone mets in shrinking them.

Yes, if there is any localized pain to the lower back or hip region, radiotherapy is definitely going to help your father. However, if there is generalized diffuse body pain, all over the body your father might need systemic treatment. Lu-177 treatment is generally reserved for the castrate resistant prostate cancer (mCRPC).
