good morning everyone. I’m 78, had all of the meds including 4 doses of Pluvicto. I have terrible pain in my right hip down to my knee. My PSA had come down to 10 but after the last dose of Pluvicto it’s back up to 17. I live on 3 oxycodone per day and the pain reduction is minimal. I’m starting to see what the end looks like. Any suggestions from you guys? Thank you!!!!

Hi,

I’m sorry to hear of your condition. If oxy is not helping your pain, there are alternatives such as morphine, methadone or fentanyl. These will make you a little loopy; better than constant pain.

As another treatment option, you may try radium 223/Xofigo.

I like plain old morphine. I am very careful about using it only when necessary. On the odd chance that you are on aberaterone, note that it prevents codeine from converting to morphine and therefor does not work for pain.

I’m assuming you have PSMA scans planned in the next week or so? Worth reading my posts as I only managed 3 Pluvicto sessions and am in the same boat as you my friend.

Have you tried spot radiation for your hip pain? Its as very effective for my Father, his PSA only ever went down to 10. Pain meds were a trial & error to find the right combination. Oxynorm/Oxycontin & difene , then methadone worked really well.

I agree with Nickhmcg. I have had similar pain due to damage where the nerve comes out the spine. If this is caused by a tumour, targeting this tumour with stereotactic radiation might help.

Sorry you are having these pain mgt issues. Go thru your oncologist to your palliative pain mgt team. Oxy , tho fun, has a tendency to build up tolerance faster than other pain meds. You should be able to find singles or combos that can manage your pain effectively. Of course radiation for the most offensive spot for relief, then meds such as : morphine ….titrated up if necessary for comfort. Currently popular combos like fentanyl patches, methadone ( low fuzzy buzz head effects ) and a breakthrough med like a fentanyl lollipop. There are a “ lot “ of great opiate or opiate like meds and combos possible. These days, even the most persistent pains are usually well managed . Discuss this with your pallitive / hospice pain crew, they are going to be very experienced at dealing with your pain .

Hopefully, you’ll find relief and comfort real soon . For most of us there is no reason to sit around experiencing pain.
