Progression of Stage IVB PC

Hello, my husband was diagnosed last year with stage IVB prostate cancer (TXN1M1B) high grade/poorly differentiated metastatic prostate adenocarcinoma with significant neuroendocrine differentiation. A PET scan after C2 of Carboplatin and etoposide showed that the disease diffusely in the skeleton, prostate as well as bilateral pelvic lymph nodes and a few small foci in the liver suspicious for possible liver metastasis. Discontinued after C5 because the disease was progressing and began Taxotere (50 mg/m² every 2 week) and acute 6-month Lupron and restart of every 3 months Zometa. We just completed C5 of Taxotere on Tuesday which has left caused him to sleep alot. Also dealing with inability to have a bowel movement. Anyone else dealing with this diagnosis?

Hi SpoonieJ, my husband’s diagnosis sounds very similar to yours.

Last March found to have Gleason 10 with 20% de-novo neuroendocrine cells. PSA 1120.

Metastases throughout skeleton and pelvic and peritoneal lymph nodes.

He has been on Fermagon followed by Prostap and Apalutamide ever since. He had radiation to fractured hip and prostate for debulking.

His bloods this last three month have shown PSA 0.4, and this month for the first time all other blood work in normal range.

We know this is limited but are extremely pleased thus far. He is very active and doing more now than before his diagnosis. He has the usual ADT side effects.

Thanks Sailing-Todd, we started off with Fermagon, Prostap and Apalutamide, but the disease was found to be progressing. The extent of this cancer with metastatic prostate adenocarcinoma and significant neuroendocrine differentiation seems to be a very agressive disease resistant to all hormonal therapies. But, we are taking one day at a time and enjoying the time God has given us.
