fda approved the lu-177 for post-chemo patients last year. However supplies of the drug are limited. Most that are getting the drug are participating in a trial that gets priority.
You might try contacting some of the large cancer centers and ask about availability.
The supply is so scarce that Novartis has placed a hold on all 1st time patients so that those who are already on the drug are able to continue their rounds of 6 total treatments.They have built a second production facility in NJ awaiting FDA approval to begin. Hopefully by this summer.
Just back from my oncology at CFCI in Boston, and I've been informed that there is no chance for any to be available until late summer!
The supplier in Europe can't supply demand. There were to be two plants in the USA to produce it however neither has been able to get FDA approval to start production, and such approval probably won't happen until this summer. I don't know why the delays.
Result is that either I'm going to be going with Radium or with a drug study. I will have to decide today as with myPSA at 375 it is getting critical. I just have to choose which route I am to follow. And as always I don't have sufficient information to make a good informed decision.
Last week our MO said Pluvicto won't be available until fall at the earliest. The Millburn, NJ site has had quality and production issues since 2018 when Novartis acquired Advanced Accelerator Applications. This is the second time in the last year production has been halted, so they still don't have their issues resolved. There is a new plant being built in Indianapolis that is supposed to be online by the end of this year which should solve supply problems.Apparently you can still get it in Europe but prices have increased as more Americans are going there seeking treatment.

Per a recent PSMA scan, my husband has very high SUV max values in his bone and lymph mets (18-66 SUVmax) so he should be a good candidate once he can get it. When supplies start flowing again, he will have a half dose or less of docetaxil (to fulfill chemo requirements) then start Pluvicto.
One other future option may be immunotherapy trials but they want to see mets be at least 1.5 cm to measure whether the drugs work or not. Most of my husband's mets are <15mm at this point, which is good.

When I started my PCa journey with radiation in 2015 I knew nothing . I just followed instructions.
In 2022 with a contained recurrence, I knew enough to question the system. Which delayed treatment just enough so that I went from a containable disease to stage 4 PCa. 🤣
Now in mid 2023 I've had close to a year of very successful SOC ADT. And I know more about asking questions!
But I'm not a good researcher, a former Dr, or anything like that! I have a list of questions for my next meeting with my MO, but no firm opinion about the direction I'd like to take for future treatments.
I have no clear question to ask today, but I am in sympathy with your inability to feel competent when making decisions under these conditions!

What drug study? Radium is already proven. Your MO should be able to tell you if Radium will help.

I have two friends whose husbands just received their first dose yesterday. One patient received his dose in Jupiter, FL (in collaboration with an NCI-designated facility), another at JH. Neither is in a trial. Maybe supply is beginning to loosen.
