For some unknown to me reasons I was not accepted to join the Pluvicto Clinical Trial at MD Anderson. My doctor is on vacations and I am waiting what's next as the Zytega /Pred combo is not working. If Pluvicto is not available probably I will go Chemo. Meanwhile I will be tested next week with the Foundation Medicine genomic kit.

Is Pluvicto ready available?
If you are eligible for Pluvicto per FDA criteria, then you don't need to be in a clinical trial. The one factor that may exclude you (or require an exception) is no prior taxane chemo.Have you had a PSMA PET scan such as Pylarify to confirm PSMA avid metastasis? I think a PSA of > 2.0 may also be a selection factor.
No, it is difficult to get it in the USA at this time. If you did not have chemo you do not qualify to get Lu 177 PSMA treatment paid by insurance in the USA.
There are other clinical trials for Lu 177 PSMA in mCRPC:

You could qualify for other clinical trial.
You could get Lu 177 PSMA treatment abroad, if financially possible. Dr Lenzo has Lu 177 PSMA I&T in Australia. Dr Hartenbach has Lu 177 PSMA available in Austria but it is much more expensive than Australia. They may have it available in Germany and in India. It is less expensive in India than in Australia and in Europe.
My Dr says that one new treatment after increasing PSA is to replace prednisone with dexamethasone. How well it works and for how long I don’t know.
One option is to do chemo, then try Xtandi after. Xtandi might work for a while and also is known to enhance PSMA expression. You could get the PSMA PET to see what the expression looks like when you get closer. Hopefully Pluvicto will become more available soon.

Pluvicto will be the 12th line of therapy for my husband. We were told it can be up to a 6 week wait in Ohio. He will be the 7th patient at a major cancer hospital.
I am in a similar situation and my oncologist thinks that Pluvicto would be the best next step. There is one trial at UCSF that is supposed to be recruiting this month. It does not require prior chemo. However there is no way to determine if you qualify until they "recruit" (or not) you.
If you are a vet the trial is ongoing at the SFVA hospital.This is the contact person for the trial at UCSF if you are a vet and are interested in the VA trial I can find the information but Darcy might know about it as well. She is not good at getting back, must be very busy.

this link to clinical trials (if it works for you) shows number 4 and 5? number 5 says recruiting so maybe it just started? i will look further and hope to hear back from Darcy tomorrow? I can't tell one study from another I do not think no 5 is the one i am waiting for.
Pluvicto is available but limited. There are protocols involved where you must first do this, then that, then do the other thing prior to receiving Pluvicto treatments. Second supply is limited AND not every facility is offering the treatments. I think your doctor and a little research you can come up with a game plan.
My husband is scheduled for his first Pluvicto on Thursday in Gilbert, AZ at Banner. Like Rondief said...first do this and then that. After Zytiga then Xdanti failed he moved on to docetaxel. PSMA PET was done in June. It does seem that Pluvicto is starting to become available.
If you are still looking to get it, here is the best way I found. If you call the manufacturer, Novartis (1-888-669-6682) and ask for the AAA PatientConnect department (1-844-638-7222) who may then connect you to Advanced Acclelerated Applications (1-646-799-6488), they can give you a list of places currently administering Pluvicto in your state just by looking at your phone number you are calling from.
I called one of them (one hour away from me) the end of October, had an appointment with a Hematology Oncologist there 2 weeks later (mid-November) and a consultation with the Radiation department there the same day. I now have my first Pluvicto treatment scheduled there for mid-December.
Of course, as others have mentioned, you need to have jumped through all of the hoops first (tried all of the hormone treatments and chemotherapy and show that the cancer is resistant to those and had a PSMA PET scan that shows it would work for you).
