MRI after radiation and apalutamide and Lupron

Found 2 positive lymph nodes below the Illiac Artery 6 years after Proton Radiation.

Started Lupron in May of last year. Abiterone failed for me due to fatty liver. Been on Apalutamide ever since. Radiation in November. My numbers have been great since my radiation. My PSA has been <.02, and my testosterone has been 0. Its been 5 months since Ive finished radiation.

I scheduled MYSELF! for an MRI.

I just want to check and see if there is anything there. I don't know if this makes any since at all!!

Its expensive and my MO acted like he didn't think I needed it but he consented.

Im scheduled April 25 to get it done.

Given the above, does it make any sense to go ahead with the MRI......

An MRI (full body?) is a really bad idea. It will detect things that will never be of consequence and will cause needless anxiety as you investigate a lot of unimportant stuff. Yes, it may find something important, but the odds are low compared to the anxiety caused.

Annual checkups and periodic colonoscopies are a much better idea.

How did you detect the two lymph nodes? You were or were not on ADT when detected? What was your PSA when detected? So you had stereotactic radiation on them and just them? You originally had Proton radiation to just the prostate?

originally had PT radiation to my prostate. 6 years later had the recurrence. Was not on ADT. PSA rose from .08 to 1.98. Found to positive lymph nodes. Could not determine gleason. Pathology same as bfore. Had 28 IMRT treatments.

So you aware that ADT shrinks PC, and while on ADT and with a PSA <0.02 and testosterone of 0 your PC is currently asleep. If you're looking to see if the radiation killed your PC you'll need to come off ADT at some point allowing testosterone to rise and see what happens to your PSA. You might want to get a even more sensitive PSA tests such as LabCorp ultrasensitive which measures to <0.006

The best MRI is in the Netherlands but at <0.02 probably not going to be useful.
