LU177 not working for me

On the day before my 3rd infusion of Lu177 PNT2002, my bone scan shows increase in size of my old mets and numerous new mets. It is obvious that the Lu177 didn't work in my case. Today I have declined the 3rd shot. Meanwhile the PSA kept rising from 23 to 41 in 4 weeks. What next??

Thank you for your input. I am waiting now for a second Liquid Biopsy in order to proceed with the next phase, probably Chemo.

You can do both (and Provenge).

I would request a biopsy (direct if possible) or a liquid biopsy to do studies of circulating tumor DNA, even circulating tumor cells and see if it is possible to get info which could guide therapy with olaparib or similar drug, keytruda or similar drug etc.

If I understood correctly your profile you never had chemo. If this is correct then discuss having chemo. If you had chemo with docetaxel, you could discuss rechallenging with cabazitaxel with or without platinum compounds.

Then there is Xofigo alone or in combination with anti androgens or even chemo.

There are multiple clinical trials, some of them use drugs called protacs which could destroy the androgen receptor. Arvinas 110 and 766 are 2 example of these drugs.

Then there are immunotherapy trials with BITE compounds or CAR T Cells and many others clinical trials.

If you could, consider to consult in a center of excellence to get oriented in your therapy including clinical trials.

Sorry to hear of your failure! I will get my first lab results after my first infusion (Feb.) in a couple of weeks. Don't give up, whatever you do.

I’m glad you got the scan and saved yourself the toxicity of the third treatment. Sorry it didn’t work for you. I’ve nothing to add to the suggestions above. This is all above my pay grader. Hopefully you find some success w xofigo or some good trial options.

very sorry Olie. I had similar results. Dose 2 showed mixed results. Failed after 3td dose with bone progression. Very disappointing since I had high hopes for the LU

Doing chemo now
