I have resistant metastatic prostate cancer. In spine and ribs.I have been on Zoladex for just over 2 years. Now on Pluvicto as I have exhausted most treatments. As Zoladex has not worked I'm thinking about stopping it. Any advice?

It will make things worse. CRPC has much more receptors for testosterone than CSPC.

You never stop Zoladex. It always works - reducing your testosterone. Your cancer and pain will get much worse if you stop.

Are there any studies that support continued use of Zoladex when castrate resistant? Why not intermittent use ? I note my FDG scan showed that I have only PSMA avid cancer, hence Pluvicto.

EVERY study of castration-resistant PCa uses Zoladex (or similar). Intermittent is NEVER used for castration-resistant men. One of the changes that occur in castration resistance is the amplification of androgen receptors - it makes your PCa much more sensitive to even the slightest amount of testosterone.

The way I see it you have both; CSPc and CRPc - the Pc doesnt a suddenly become resistant - the resistant tumours are a new mutation that has jumped out of the box. So your Zoladex is continuing to do a good job on the CSPc whilst the CRPc is developing. (Dont kow if this is science supported but it seems likely to me!!)

Quite so - but good science is guided by logical observations - and starts as an hypothesis which then has to be proved - or disproved!!
