MY dad was not accepted for Lutetium (177Lu) treatment since he's not had chemo. What is SPLASH? We live in Michigan. Other options?

My dad is a VERY full of life, very vital man of 87 years YOUNG. Stage 4 Prostate Cancer. Went to Cleveland Clinic...was told he was a candidate for Lutetium. Was told our doctors here are great. Went to Karmanos, met with Oncologist - 8 hours later, she loves my dad...he loves her...

Today we get the notification that he is NOT a candidate since he has not had chemo.

EVERYONE knew that before.

What are the other options.

Can he pay out of pocket?

He was waiting to hear if he would do Arm A, or Arm B...and now he supposedly can;t do either.

HELP.

He has a LOT Of life in him and has an amazing mindset and will to live.

Please share any tips/tools/ other treatments .

Here are the clinical trials for Pluvicto and similar radiopharmaceuticals in the US. The trial of Lu-177-PSMA-I&T has a site in Troy, Michigan. It is the only one your father may qualify for:

How can I find the one in Troy, Michigan and how do you know this! We live 10 minutes from Troy, MI.

Call GenesisCare in Troy,Michigan and ask to speak to the lead investigator of their trial of Lu-177-PSMA-I&T.

WOW - YOU are an amazing human to have sent me this. Praying it works, but even if he can't get this - we are a STRONG family with a lot of love and support and my dad is an AS$ kicker full of life and want's to live - so I feel that is more than half the battle with cancer. many on this site, including myself, have gone abroad for Lu-PSMA and similar treatments when not fitting FDA requirements. I went to GenesisCare AU in Australia. Many have gone to Germany, India, Finland. Of course it is self-pay. But they are more experienced than in the USA.

Otherwise it is a clinical trial as noted. Or game it if one is comfortable doing so: get one dose of chemo and then choose to discontinue due to intolerable (Grade 3) side effects such as asthenia (fatigue and weakness) or emerging peripheral neuropathy. Just stating choices, not recommending.

Write to the University of Heidelberg. Ask if he can do LU-177 and AC-335 combination.

Hi Dawn,

Your dad sounds like a wonderful man. If you are wondering about the SPLASH trial, it is a study evaluating metastatic castrate resistant PCa treatment using Lutetium-177 PNT 2002 PSMA therapy after second-line hormonal treatment. The official study title for an internet search is "Clinical Trials.gov: NCT04647256. Prior chemotherapy is not a prerequisite. Don't delay, I understand only 400 men admitted into this trial world wide. Good luck!

I had 2 infusions of LU-177 without chemo & no ADT for probably 2 years prior & it reduced the PSA from 19 to 1.6, so it worked & gave me 2 years before it came back to 18.

I believe if they had given me a third infusion of LU-177 it would have saved me & rid me of this terrible disease but that is how it works here in OZ.

My understanding is that the" chemo first" is a Medicare imposed rule. In my case we had skipped Docetaxol because I had a mixed tumor type. When I appeared to be good candidate for Lutetium177 treatment, I was told that for Medicare and my Advantage plan to pay, I would need to have chemo first. I am halfway through the chemo and have three more infusions. Not an easy course.

Please Dawn don’t be discouraged about this . Another window will open ..He has your love and support…I was unable to be near my dad when he got pc . Cherish the time with him . That what counts . He must be an amazing dad to have such love .take care . 💔😔

Dr Scott Tagawa at Weil/Cornell in NYC is running a Clinical Trial on LU177-PNT2002 ( LU-177-I&T) that may still have openings Everyone gets LU-177 w/o prior Chemo.

Check Him out. I was just up last Thursday for initial office (not treatment) visit.
