I was diagnosed last December. Some bone mets, no soft tissue issues. PSA 365. Firmagon for a month, then Lupron since. Also Zytiga w/Prednisone. PSA went down to 1. Two months ago my liver enzymes started to rise. Took a one month vacation from Zytiga. Liver enzymes came back to normal. Resumed Zytiga 2 weeks ago at half dosage. The Liver enzymes are starting to rise again. My MO is considering moving to Xtandi. What would be the side effect difference?

Xtandi vs. Zytiga
SEs for both drugs vary enormously between different individuals. You can find cases on here of some guys who've had really bad SEs on Xtandi (to the extent of being a virtual invalid) and other cases who've just had some mild fatigue and hot flushes. I started Xtandi last Sunday in addition to the Prostap I've been on for over 3 months. It's early days but so far, I've not noticed any obvious extra SEs. I'm sure I'll get some as time goes on though! There is always the option to reduce the dose if you get bad SEs too.
Xtandi can deliver , and do it long term. …. the vast majority of guys have light to moderate SEs. Considering the potential benefits, you can try Xtandi and switch if you feel like you need to. Got my 48th month psa results back this morning and “ still “ <0.1 undetectable.

Give it some time. If you switch to Xtandi, be sure to slowly wean yourself from prednisone. Most of the side effects are from ADT.
