Hi everyone, my Dad started having hard time urinating, he needs to push to get it started and has a weak stream. The PSMA scan back in June 2022 did show some residual cancer in the left side of prostate gland but we were told that the radiation and Zytiga are still working and will eventually get rid of it. PSA keeps going down, currently at 0.07. He had a TURP one year ago when he was diagnosed with PCa. We have an appointment with urologist tomorrow. I am hoping is just some scar tissue from TURP and not the disease coursing the problems since PSA still going down. Is it normal to have scar tissue only one year afterTURP procedure? Can they do dilation to remove the scar tissue? Is there any test/scan that we should ask for to rule out that the blockage is caused by cancer? Thank you again for all your help. Has he tried taking Tamsulosin(Flomax)? It worked wonders for me.
In addition to the Tamsulosin, what worked for me is over the counter - AVO - bladder control.
his Urilogist will be a cystoscope and take a look. Could be scar tissue or radiation cystitis. Guessing the the prostate loned are enlarged and pushing against the ureatha. An Urololift may be the answer it had better results than a TURP
One thing that he can try is to take multiple Sitz baths. Each should relax his enough that he can pee away in the water. It won’t hurt him and it will give him temporary relief. I did that after my radiation in 2003.

Actually 6 months after a successful TURP, I got scar tissue, elevated by Creatine to point, Sloan hospitalized me and tried catherization. After many difficult to insert catheter, they finally got one in. When creatine normalized, they removed catheter, I my flow was normal. I guess the difficult insert cut through the scar tissue. Cystoscopy may do the same for your father, Good luck.

I have had intermittent weak stream and pain severe at times. It is so unpredictable that it must be from multiple causes. I had radiation and am sure i have radiation cystitis. I had a urolift that worked until it was removed by an emergency TURP.
I just never know when I will be able to pee normally but at least I have been able to insert a catheter reliably so I am not freaked out about having to go to the ER where the doctor really fucked me up.
Most recently I had severe pain on urination and major urgency peening just a little each time. I was pretty sure I had a YTI but my urologist advised not overtreating UTIs. I was treating them about 4 times a year. He said that no one ever died from a UTI so just let them be. Well he was not experiencing the pain. I got a urinalysis ordered by my primary care doctor and I had a raging UTI for which she prescribed ceflex. I am now peeing almost normally, woke up to pee just three times last night and almost no pain
So definitely get the cyscopy and see what is going on in there. My urologist said that it is a mess in there and he could make any determination and recommended another TURP which I will not do. TURP on irradiated tissue is a very bad idea and might very well be what is the source of your problems.
Good peeing is a delicate balance between a number of factors and for those who are compromised there can be one more little thing that snaps the back of the camel. For me recently it was a serious UTI. I listen to my primary more than I do other doctors. She is amazing. When she is insistent about something I do it.
Be sure to have urinalysis especially if you have anything other than nice clear yellow urine. Mine was very brown and I should have just said :fuck the urologist, im getting antibiotics"
I am so sorry for all the pain and problems you had to deal with, sometimes urologists are not as caring as they should be, thanks God with your PCP, so glad your UTI has resolved and you are doing so much better right now ❤️
Another tale of a bad urologist. Ignore a UTI -- really? WTH. I hope your problems resolve.
Ibuprofin or Advil helped me a lot before chemo really helped.
Tamsulosin didn't work for me either after having Brachytherapy and EBRT. What has worked for me is Duodart or Doubluts which has Dutasteride in it, and Dutasteride is known to be beneficial in combatting cancer. You have to manage it and when you take it and I will probably be on it for the rest of my days.
Good responses above, only one thought to add. Before my diagnosis I had problems with frequent urination, but only small amounts each time. Then, I couldn't urinate at all and had to go to the emergency room twice for catherizations. The second time my bladder was in danger of bursting. Turns out that peeing and not emptying my bladder had created the perfect conditions for bladder stones. Some stones got stuck at the point where the urethea and bladder connect.
