Hi guys/ladies. I am writing this post for my son. We need your thoughts. He is a 49 yr. old male, who had RALP for pc on 3/7/22: Path : staging, pT2, pN not assigned (no nodes submitted); gleason 3+4=7, patt. 5 (less than 5%) Tumor Quan. 5%; multifocal; neg. epe; pos. sur. margin at left apex. (unifocal, limited,0.1cm.(1mm). gleason 3+3=6 at margin. Neg: LVSI, PNI, SV. Now PSA raising within 6 months of surgery: 5/28 (uPSA .061); 5/30 (reg PSA .07); 6/25 (uPSA .078); 7/18 (uPSA 0.104) On 7/22/22 met RO. RO Recommends SRT whole prostrate bed + pelvis LNs. w/ IMRT or VMAT (over 33 treats) plus 6mons. ADT. PSMA-Pet sched. 8/19/22. ADT sched. to start @ 8/20/22. Simulation and SRT sched to start thereafter. We would greatly appreciate any views, tips, pointers or opinions from any interested party. Many Thanks.

His low Gleason score and volume are very favorable. You have reason to be very optimistic.
Why would PSMA-PET scan take place post ADT? With such a low G score and otherwise, why wouldn't they want to see what's going on without obfuscated results due to suppressing the PCa with androgen drugs...

I find that plan to be optimal treatment. When PSA is that low, 6 months of adjuvant ADT, rather than 18+ months appears sufficient. Verify the treatment plan is for adequate dosing to PLN fields extending to aortic bifurcation. 👍👍

Take probiotics like CVS 20 billion dose prior to and throughout radiation to protect lining of intestions.
Stop taking any anti-oxidants during radiation.
You need to drink water to fill the bladder for each session. Exactly the same each day.
Get two 20 oz tall bicycle water bottles. For me I drank the first bottle over 15 mins beginning 45 mins before my radiation start time. Then I sipped the second over the next half hour drinking about 3/4 of it. That filled my bladder.

We are almost same . Was also diagnosed at age 49 psa 7.41 gleason 3+4 pt3b . Had prostatectomy and adjuvant rt. Psa undetectable since rt for 2.5 yrs. 👍
