Does anyone else have a psa that fluctuates in the .20, .30, .40s while on Zytiga. Is it common to have some PsA when you still have your prostate?

Not only is it common, you have to have some PSA when you still have your prostate. Bounces are common and expected.

So that’s ok on Zytiga doesn’t mean it’s failing cause it’s up and down

Isn’t the (benign) PSA produced by prostate still androgen-dependent? So that, while on lupron/zytiga/etc, it should be suppressed?

Benign PSA has nothing to do with the AR. It leaks into the serum from leaky blood vessels.

Didn’t know that, thanks. This makes me wonder, since I still have my prostate, how then would my PSA get down to 0.02-0.04 range while on lupron/zytiga? Perhaps I’m misunderstanding.

When blood vessels are patent, as they are when prostate cells are healthy, they don't leak PSA into the serum. It is only when blood vessels are leaky, as they are with BPH, prostatitis, or PCa, that PSA leaks out into the serum.

I've been on Lupron (only) since 11/2018. During that time, my PSA has been mostly 0.6-0.7. However, I had readings of 1.3 and 1.4 in May and June 2020, after which my PSA began a slow but steady drop to an all-time low of 0.5 in April 2021. The last 15 months 0.6 all the way except for one 0.7 reading. The last one was 0.66 in late June (Quest began reporting in hundredths instead of tenth as in 12/2021.

I'm metastatic to bones and still have prostate.

Same here, except I’m only on zytiga. .6 and .7

Only Zytiga, no Lupron? I mentioned that idea to my MO, but it'll take half a dozen tries before he loosens up. That's my new method; just be a grain of sand.

I did lupron for 18mo and literary was losing my mind. No chance of me going back on that unless it’s imminent death.

I hear ya. Lupron hasn't bothered much. So I've just stuck with it.
