Psa 0.26 the results of ga psma pet scan are ABDOMINOPELVIC NODES: PSMA avid right internal iliac lymph node, image

263, 0.5 x 0.4 cm, SUV 3.3.

I had davinci 2/2014. Salvage radiation 9/ 2015. Psa no longer undetectable 6/2019. Original Gleason 3+4, T2b.

I'm having consult w MO on Friday to discuss next treatment. I would appreciate any thoughts, recommendations of questions I may ask.. I am brca 2 positive. Thank you .

I would discuss having whole pelvis radiation (prostate fossa and lymph nodes) and 2 years of ADT plus abiraterone as per the STAMPEDE trial

This treatment offer good chances of long term control of the cancer, and if there are not distant mets (the PSMA PET/CT does not see any lesion less than 4 mm) , it could be curative.

The node is not in the prostate fossa or the bed of the pelvic floor, from my understanding it would be higher on the internal iliac lymph node chain.

It is not consider a metastasis. The cancer is N1 M0 according to the PSMA study. The cancer still is in the pelvis around the internal iliac artery.
