Lot's of choices depending upon your specific situation. If chemo naive then docetaxel or cabazitaxel, potential for AR inhibitor like enzalutamide( although some say limited potential), Provenge and lastly Xofigo or Lu-177. Also various clinical trials ongoing. Depending upon your variant status maybe one of the monoclonals like Keytruda, etc. So lots to look into. I'm approaching the same decision with abiraterone starting to wane in it's effectiveness. I have switched to dexamethasone from prednisone and got an 8 point drop in PSA. Not a huge decrease, but at least it's not going up. My MO is leaning towards Xofigo or chemo, pros and cons to both. Talk it over with your doc. Lots of life left to live!
Xtandi would be a good next step.
When I asked my former oncologist (nothing wrong with the oncologist except for leaving for a new job) what my future treatments were likely to look like (currently on Lupron with Zytiga + Prednisone) I was told that for folks in my situation, it's almost always going to be some combination of "Shot & A Pill." "And there are always new studies and treatments coming out, so what you would get six months from now isn't necessarily the same as what you would get in two years."
