Thanks to all for your response. As a follow up I am posting the results of my PSMA scans hoping I can get some direction for what is next.

1. Interval disease progression I the region of the left seminal vesicle with now a 4 cm area of soft tissue thickening and intense radiotracer uptake, extending to the base of the prostate and possibly into the perirectal space as described.

2. Several new areas of nodular perirectal involvement as well as a new right medial common iliac lymph node with intense activity.

3. No evidence of active disease outside of pelvis.

4. No evidence of increased bony activity with scattered sclerotic disease stable as before.

Is docotaxel the route you would take or try Zytiga? Had some success with Xtandi. Thanks for your help. Have call with Dr Delsom of Siteman Advance Cancer in St Louis for his opinion.

Best to alternate chemo and hormonals. You did Xtandi, so docetaxel is next.

Why not? When it gets approved, they will require docetaxel first.
