First effects of LUPSMA treatment

After the first injection, how long does it take to feel the first effects of the LU177PSMA treatment?

If you have bone mets, bone pain should be reduced after two weeks.

I was asymptomatic before treatmemt and after treatment so have no effects to report other than spect scan day after showing where it had landed and PSA tests every two weeks going down ( but I am on Degarelix and hormone sensitive so no idea what’s doing what.

I am due full suite of comparative scans when course finishes(my course is an unusual 3x Lu-177 and 3 x Docetaxel)

I consulted with Finland and as I have no curative standard of care pathway they have authorised early Lu-177 use. They are playing it infusion by infusion and watching the response. So far so good.. my PSA 25th November 2021 was 17.6 with multiple mets to spine. They started me on Degarelix and Lu-177 every 4 weeks for 3 infusions. Bloods last week showed PSA of 0.46 18 days after 3rd infusion and all other bloods in norms so signed off for chemo 1 on Monday to mix it up. I appreciate this is going to be tougher than I’ve experienced so far but it makes sense to me even if off SOC. There appear to be a number of countries you can get Lu-177 when hormone sensitive but not here in the UK so a commute was necessary., The Docrates hospital in Helsinki are a specialist oncology hospital so you can consult holistically with all treatment possibilities on the table. Saying that my London MO was happy to have organised the 3 chemos but I decided to complete the Docrates plan as I have been impressed with the staff and the details like zapping my breast buds with radiotherapy just before the Degarelix, the ice/ hydration protocol after the Lu-177 ( not even a mild dry mouth experienced). For Mondays chemo the cold cap/ gloves/slippers are done as standard.

Can you also get Provenge at Docrates while HS?

I haven’t asked and it was mentioned. I have only heard of provenge in the USA.

And what do you have to pay for the Lu-PSMA treatments since you are from out of country? Thanks.

Initial consult/investigations/scans and 1 Lu-177 was around 14,000 euros and 2 and 3 around 10,000 euros each

The chemo is much cheaper!

My husband is also hormone sensitive and has never taken ADT. He had prostatectomy in November 2018, gleason 9 and the PSA was 11.8. Six weeks after the surgery his PSA was still 2.46 and a ga68psma petscan showed mets in the pelvis, aorta bifurcation and so on. He's got 3 Lu177 treatments in Vienna in 2019: march 6th , April 4th and August first ( he couldn't get the third treatment earlier because of an inflammation in the pleura, after several surgeries he had before on this place for a thymoma). The PSA in March 2019 before the first Lu177 treatment was 4.40. It dropped to 0.34 in May 2020. Then it started to grow and in July 2020 a psma petscan showed 3 new mets. One in the cervical c7, the first in the bones. In October 2020 he's got 5 sessions of cyberknife, but it did nothing at all. So the PSA continued to grow and a new ga68psma petscan showed another new mets in the spine and 2 mets in the pelvis. He's got three other Lu177 sessions in 202, in Homburg in Germany in October 6th, in December 7th and just now, on the first of march 2022. A ga68psma petscan on January 25 ( after the first two Lu177 sessions in Homburg) showed that all mets after the three treatments in 2019 have disappeared and the new ones, in the spine and in the pelvis have almost disappeared. To see if the third Lu177 treatment killed them all, my husband will get a ga68psma in three months. Before the Lu177 in October, the PSA was 6.14. On February 28th it dropped to 1.86, after two Lu177 in Homburg. I hope it will interest you.
